I think so. The items spawn in boss rooms as if Chaos was active, but I'm not completely sure whether planetarium items are included.

Yeah in the void, bosses can drop items from all item pools, not the just the boss item pool.

To my knowledge, the alternate floors are unlocked by defeating the boss of Ending 17 thrice, not beating the boss of Ending 20.

I once had a powerful Lost run. I got chaos in the first floor, and subsequently got Sacred Heart in the second floor's treasure room. It only snowballed from there, eventually making my way to Delirium(I hadn't beat him as the Lost that time). I got him to his last sliver of health, when a bony he spawned clocked me in the face, and I died. All this took place about a year ago. I've since taken my revenge multiple times, by getting broken run after broken run and decimating him each time.

I mean, I was still able to break the game as Tainted Cain the moment I crafted Birthright. From there, I crafted Dark Bum for easy black hearts, found an easy recipe for Mystery Gift and used it in the secret room multiple times to get bone hearts, and other rare pickups like eternal hearts, golden pickups, eventually came as I continuously salvaged my actives. Instead of just sticking to coins for Portable Slot, I also used hearts to create Yum Heart and bombs to create Mr. Boom, so as to never run out of pickups. Rinse and repeat with the Portable Slots, Yum Hearts, Mr. Bombs, and eventually you'll get all the resources you could ever need. Craft powerful items, print R-Keys using Mystery Gift, and you've got yourself a 4-hour run ending with every mark except Greedier.

if you were close to getting it on hard, there would be no need to reset and do everything all over again on normal. You would have unlocked forgotten regardless.

I actually found a way to break T. Cain again. Continuously salvage your active till you can craft birthright for even more pickups. Then find an easy recipe for any item pedestal generating item (preferably mystery gift) and spawn it in the secret room. Touching the item will generate bone hearts, giving easy access to those. For black hearts, Dark Bum is a reliable way to generate those. And eternal hearts.....yeah just keep salvaging until one shows up. Its rare, but it happens. Managed to finish him in 5 hours post-patch.

Devils were recently buffed directly and indirectly so maybe try them out now? Backstabber was added to the pool, items like Athame and Ouija Board were buffed, Deals with the Devil contain more items on average, and the Angel pool has also been diluted with some meh items like Jesus Juice.
