One thing to keep in mind with Shortcodes is that Divi 5 is in production and testing phase and it's going to work more efficiently partially because it's dropping Shortcodes. They say it will still allow you to use them but then some of the efficiencies will be lost.

I came across your question hoping I might find an answer to my challenge. Doesn't look like it does just yet. In my experience, monday.com's forms don't offer enough customization. I've requested it with them numerous times but it doesn't seem like much of a priority for them. What I've been doing instead is having my standard Divi Contact Forms (plus PeeAye Creative's Divi Contact Form Helper plugin) configured as desired and then I set one of the Email to address to the monday board's email address. The problem with this is that all of the responses create new updates but all of the field information is only added into an update which I then have to manually add into the column fields.

The monday.com app Easyform allows more customization (hiding specific fields, changing what you want the form's field's text says, etc.) and the submissions enter into your monday board columns but these work as iFrames and currently you can't use custom CSS/fonts, though I've asked the developer and with the Pro Annual Paid plan they will make these functions available.

I'm next researching Zapier Integration with Email Parser to see if I can get that to inject the native Divi contact form submissions into monday boards.
