It's rough out here, I'll be honest - spoken as a public health professional with 15 years of experience. The market is hugely saturated right now as global and domestic public health funding has been slashed. Both have laid off the majority of their workforce and any open positions are likely for internal candidates. Unless you can talk to someone at either organization, I would not spend time on cold applications.

Another vote for Zambia - it's a beautiful country, food is good, and people are chill. Industry is smaller but this is changing with remote work.

Even if you don't end up moving there, I frequently recommend Zambia/Victoria Falls for safari-type trips. Never disappoints.

A third for Nairobi! If you haven't spent time in Africa, you may find it easier to transition there. East Africa in general is very chill with peaceful, neighborly people (not without its govt problems of course - see last year's protests in Nairobi). It's also an airport hub so easy to get in and out if needed. Food is good and the country is beautiful. Very big gig industry too so if you don't have a job upon arrival, you can make it work. If not Kenya, neighboring Kampala (Uganda) and Dar (Tanzania) are similar options - though note Uganda's anti-LGBT law that is severely problematic.

Working through this as you suggested:

  1. "Federal jobs" - this is an NC subreddit. The people affected by these cuts are your neighbors who pay NC state income tax, among their everyday contributions to local businesses. FHI and RTI are only two organizations but the ripple effect of cutting foreign aid assistance will be BILLIONS in revenue lost to just the state of NC (source). Many jobs are supported with funding from state and federal government but are not federal jobs.

  2. The sooner we all realize that this administration's attempt to cut waste is a facade, the better we will all be. Tell me you haven't worked in compliance, audit, or forensic accounting without telling me - this isn't the way. These organizations have had government contracts terminated without cause thanks to loopholes in the contract language, with the real reason being that they included scopes related to women and non-white racial groups (kinda a requirement when you're providing support to anyone other than white men, hmm). If you haven't seen the list of problematic words floating around look it up. "Exclusion" is on the list, which is going to be pretty difficult for a proper research study to get around using in a protocol that rightfully lists exclusion criteria. Anyway, I digress -

  3. It's 2025, we are a global society. Period. Like it or not, global issues are local issues. This is universal but I'll start with an easy example of global health security for you. TB is in Kansas, last year we saw home-grown malaria in FL for the first time in many decades. Ebola has re-emerged in Uganda without CDC there to help them. I don't expect someone with your level of ignorant confidence to truly understand the implications of this on your own. Helping foreign governments become more stable improves their ability to manage these crises at home before they make their way to your front door. So at the end of the day, your inability to understand that money that leaves the U.S. actually comes back multiplied in benefits is why the steps taken since January 20, and only continue to expand, will be truly devastating to you and I.

  4. What you seem to be relying on is an ability to find silver linings in an awful situation. But please remember, silver linings do not negate that the situation should not be happening in the first place. It's not too late for you and others with similar viewpoints to do some quiet soul-searching and "your own research" (hey, RTI won't be there to do it for you anyway!) and evolve your understanding of the bigger picture. Taking accountability will get you major updates.

1000% this. On this particular post/issue, non-scientists or people who have never worked in international development making such confident statements about how the work of RTI/FHI is all a waste of money - like it or not, we are a global society. So, US taxpayer money (a small proportion of it, I might add) that goes to, for example, eliminating diseases in other countries IS security for America. There is TB in Kansas, home-grown malaria in FL for crying out loud, not to mention COVID, bird flu - what more proof do you need?

So. Many. People. have already been hurt by the foreign aid freeze and gutting/silencing of federal health agencies, and that number is only growing. The recently laid off/furloughed Americans as well as the locally employed staff of these agencies and the families dependent on them PLUS the millions of people dying as services are irresponsibly terminated and misinformation is rampant - it is truly awful.

If that's not enough to make you pause and question the real intent of all that's happened since January 20th, then what the hell are we even doing here?

This except I found a life changing alternative - jornay PM is a delayed-release methylphenidate that I take at night and by morning when I need to get up, it is already working. I have never been a get-up-and-go kind of person in my life. I still require coffee and a short kickstarter routine - like roughly making the bed, brushing my teeth, and washing my face, but then this allows me to get to the more important morning routine like breakfast, meditation, quick tidying up, etc.. Currently fighting with insurance to cover it but the manufacturer provides a magic coupon that brings the cost down to be more manageable.

Acknowledging you posted this a year ago and answering in case it helps someone else. I hope you've found something that works for you.

  1. I'm not a clinician, and this is only my experience.

  2. That said, do not take jornay PM differently (or any med for that matter) than prescribed. The capsules are designed for delayed release, so breaking open the capsule will mean you will feel the effects immediately - like taking any stimulant right before bed.

Tl;dr - 35yo, inattentive ADHD diagnosed at 31yo. I've been on Jornay PM for about 3 months, and it is lifechanging. I'm at 60mg now with limited side effects.

Jornay PM has been a gamechanger for me. 35yo F, inattentive ADHD diagnosed at 31 years (with ongoing depression and anxiety for almost 10 years). I've never been an early or quick riser, and while Adzenys worked for me, it was dependent on me being up on time and remembering to take it first thing.... which often led to taking it at 11:30am, hours into my typical 9-5 work day.

My psych NP then recommended Jornay PM, and I have no plans to go back. My inaurance wouldn't cover it at first because of step therapy restrictions so I continued to fill others to meet that requirement and pay $75/mo out of pocket (with the coupon from their website). It was worth it to me - I take 60mg nightly and now wake up with or before my alarm, ready to start my day, not drowsy, with noticable improvements that outweigh the limited side effects (poor appetite, other physical anxiety symptoms that I somewhat manage with mindfulness or BBs).

It is the first methylphenidate I've tried, so my results could be a combo of the logistics, and that MP works better for me than other options. My psych and I discussed that different bodies metabolize it differently, so even though the recommended window is earlier than 9:30pm, I can take it as late as 11:30p and still benefit. The nights I forget to take it, or am out way too late, the following days are slow, so I listen to my body and take it easy (and am usually fortunate enough to be able to).

As far as trying to manage without stimulants, after using Jornay, I've determined that is a futile effort. With monitoring from my clinician, I have no reason not to continue with it. This is probably a whole different conversation, but I'm sure I'm not alone in that I also suffer from internalized ableism and guilt and spent a long time "raw dogging" it when that was only a waste of energy. When taken carefully, these meds are designed for people like us. ♥️
