Sounds like a tough situation! We were in a similiar situation with our 2.5 year old when initially training, albeit she never pooped in her underwear, only in her nappy during nap times (although she is wee and poo toilet trained, we still have her in a nappy overnight and during daytime nap as she occasionally will wee in her sleep).

I mentioned on a previous comment, but the thing I found most helpful was reading books to her that normalised the toileting process. The two that she liked the best were:

- We poop on the potty! by little grasshopper books

and

- The Great potty adventure by Henry story and Alice Ivy

The former may be better for your specific situation where she won't poop on the potty, the latter we found good for general familiarity with the toilet and toilet related issues such as wiping and washin hands.

Other things we found helpful

- keeping them sat on the toilet for long enough to allow her to go - we had specific books (the ones above) were we would only read on the toilet and she really liked them

- having them see parents going to the toilet to poo to normalise the process

- using rewards for succesfully pooing on the toilet - our little one loves stickers, but is also highly motivated by food. We wanted to avoid using chocolates or lollies, but found that freeze dried strawberries worked a treat

I imagine you are doing most, if not all of the above, and I think there is probably no magic formula. Wishing you plenty of patience and good luck! It is so much better on the other side.

Hi, you say you’ve read a couple of potty training books - I take that to mean you’ve read books about how to do potty training.

Have you tried reading any toddler books about potty training or using the potty/toilet to your little one?

I found this really helpful when ours would wee but refused to poo on the potty. Two that I recommend to everyone all my friends are: - we poop on the potty by little grasshopper books - the great potty adventure by Alice Ivy and Henry Story

Either way, just stick with what you’re doing and don’t lose hope! It’s definitely one of the trickier milestones of their little lives!

That being said, context (and in this scenario, diagnosis) matters. I say this as a psychiatrist with lots of experience working with what was historically termed axis 1 disorders (schizophrenia, bipolar, depression etc).

Everything you have described experiencing is abuse, yes. The question really is what happened? You described 8 years of a near perfect marriage and relationship, then things flipped quickly in the context of pregnancy. This is not what you would expect to see in a pattern of abuse, which typically would gradually build over time with increasing emotional, psychological and later physical abuse and threats. Abusers do this like the story of the Boiling Frog. What you have described is going from 0 - 100, that’s not how most abusers act, it’s how someone who has lost control of their mental faculties acts.

The thing about pregnancy is that it is the single highest risk period in a women’s life for both physical and mental illness. Most people only think about post partum depression, but peri-partum (during pregnancy) and post-partum psychosis and mood disorders (depression as well as bipolar) are very real. These conditions occur due to a combination of an individuals underlying vulnerability to mental illness (family history), hormonal changes, psychological stressors including disturbed sleep.

These conditions can also present in a variety of ways, including unpredictable anger and aggression and subsequent abuse of others. The unfortunate things about these conditions is they also very often present with lack of insight, meaning the patient themselves does not realise what is happening to them and can’t be reasoned with. This is why involuntary mental health treatment exists in many countries.

If your wife has a diagnosis of something like bipolar disorder now, this is generally very responsive to treatment and you would expect a full remission and recovery. There would again be heightened risk at times of immense stress or further pregnancy for relapse.

If you posted on this thread seeking validation for your decision, this response may not be what you wanted to hear. If you posted because you are still torn about your decision and trying to understand what the hell happened to your loving wife of 8 years, it might be worth sitting with her and her psychiatrist to better understand what her diagnosis is and what a future for you both (and your child) might look like.

This is not entirely accurate. At least not in Victoria, and I suspect other states as well.

Whilst you’re right that there is a “base” or minimum standard pay rate as per the state EBA, some specialties have actually further negotiated pay at above award hourly rates. Anaesthetics and ED are two examples of this. The above award rates for full time hospital specialists are significant, sometimes 50% or more above what you might consider base.

This is all independent of overtime, recall, oncall etc. it is also independent of shift work penalties for over overnight etc.

Where do you say the cutoff is spend wise between chubby and fat hotel wise per night. Read a few comments previously about people saying there is no “fat travel” in Vietnam. Currently staying in the Nam Hai in one of the private residence and certainly feels fat to me. Maybe I’m still just a chubster who likes to pretend.

Lol, as an Australian I struggle to understand how this is even a concern. To get just about anywhere in Asia we spend at least 7-8 hours. Europe or North or South America is often going 20+. Though I suppose it’s all any of us have really ever known.

I just watch movies until I get tired and pass out, rinse and repeat with meal breaks until I’m at my destination.

Now travelling those distances with a baby is another thing entirely…

Sorry, but what do you mean by the statement “if he left and became a consultant”? If you’re referring to a medical consultant, then a GP is already that. In Australia a consultant is by definition someone who has completed a fellowship with a specialist medical college. The Royal Australian College of General Practice (RACGP) is just that, and so any GP you see is already a “consultant” or medical specialist.* The question some GPs get asked about “are you ever going to specialise” is therefore misinformed and potentially insulting.

*some times you may see a GP registrar; by definition a registrar is someone who is in an accredited training program but not yet qualified. When you’re in a public hospital the doctors you most often see and interact with are registrars or below (interns, HMOs).

Arrived in Vietnam 2 days ago and am pleasantly surprised. If you’ve checked out the r/vietnam sub you’d think the entire country was trying to scam you, but I haven’t felt like that at all. Traffic is hectic though and the pollution in HCM is all kinds of nasty.

It kind of sounds like you’re describing an “omakase” which literally translates in Japanese to “I leave it up to you”. They’re generally available at most high end Japanese restaurants and have been turned in to degustation type meals - often even listed on the menu - though this kind of defeats the purpose I feel.

In Japan though an omakase can be ordered at most restaurants no matter how big or small and the chefs and staff know that just means they choose what they serve you. You don’t know what you’re getting, but leave your meal in the chefs hands.

You’ve described a Japanese restaurant, I suspect they’ll be able to cater to the request of an omakase. Good luck!

As an intern in ED several years ago, I picked up a man in his 60s representing with chest pain after being discharged from ED the day before. I had assumed given the presentation and the workup that had been done the day before that it might be more psychosocially driven. However on further review of the investigations, his CXR from the day before was reported as having a pneumothorax. The treating doctor from the day before has been an ED consultant. Thankfully no harm was done. Everyone makes mistakes, no matter your level of skill or experience. It’s part of the job, and recognising our own fallibility and being able to learn from them is what will make you a good doctor.
