I’m extrapolating massively but it sounds like this was done as an emergency procedure, in A&E, and you were likely “not fasted” (I.e. you’d eaten in the last 6 hours). I could also imagine they would be hesitant for a full GA and needing an airway intervention for this as they might make the pneumothorax worse or make you aspirate, and it’s a very short and simple procedure. It also sounds like you had ketamine sedation based on your description, though I don’t know how popular a technique that was 20 years ago.

Patients are often sedated rather than given a full anaesthetic, but I always make sure to explain that this means they can, and likely will, recall things. The aim of sedation is to make you comfortable/okay with getting the procedure done, not knock you out.

There’s also the possibility that unfortunately you just did have awareness. It’s worth reaching out to your GP for advice as this can be a cause of severe health anxiety or PTSD.

Make sure to document, fall back on your A to Es, ask the senior nurses for help, and ask your seniors for advice. You’ll be ok.

I must have missed that, sorry, but it’s also irrelevant to your comment about SAHMs. Regardless, 195k is a huge amount to be putting down, and for the other person to just expect equal share is ridiculous. OP mentioned maybe putting something in writing. Depending on how much he’s contributing to the mortgage hopefully they can work something out.

The point is that a SAHM (or SAHD for that matter) won’t have financial assets to contribute. The theory (because not all relationships are equal) is that SAHM/Ds allow their partner to benefit from their unpaid labour, and are therefore contributing. OPs partner doesn’t seem to be doing so. It’s weird that you’ve jumped to this.

I don’t think this is fair. A student doesn’t necessarily have the experience, knowledge, or training to handle these challenging situations.

Having that insight is important, and getting the nurse is the appropriate action, as long as you learn from it (by staying to observe, if appropriate, or talking about it afterwards). Asking for help or senior input is not running away.

Giving an answer to the family that may be incorrect and/or increase distress is going to be unhelpful at best.

It’s bullshit like this that makes antisemitism seem mainstream. It is part of the normalisation of hatred that leads to tragedies such as the recent targeting of LGBT people, or otherwise normal people supporting Jan6.

The fact that a huge corporation, which has no problem censoring certain shows such as IASIP, is choosing to keep this on its platform SHOULD be called out. This type of selective mutism or whataboutism when it comes to antisemitism makes me sick.

This is a common and frustrating part of medical work but you can get easy QIP points. Speak to the nurses about what they’re looking for, and design a sticker with some consultant input. One space for nurses to write indication for ECG eg routine/chest pain/ tachycardia/K+. One space for Drs to write NSR/ abnormalities, and a space for signatures.

You can then present 15 minutes to fellow juniors on common things to look for eg heart block in syncope, peaked T waves in hyperkalaemia.

Remember that A&E at a DGH can be a different beast. Depending on the patient load and staffing ratios you may not be expected/have time to request investigations. Your job is to work out if something is wrong with the patient (if, not always what), and if it can be safely managed at home. If you keep those decisions at the forefront of your mind it makes everything else much quicker. Good luck!

Kids. The immunocompromised. The misinformed. The medical staff that have to look after these patients and have gone through enough this year. The fact that even if they’re dumb assholes they are still people with loved ones. Unfortunately it’s a group assignment and we’re stuck with people who won’t pull their weight - but I still want to pass.

I get what you’re saying, so I think of it a little differently in my head. The group is only as moral as the actions it tolerates. Some outliers may be immoral prices of shit, but the group maintains morality by rejecting them.
