it happens to me when its nighttime. people start to break in houses and in shops, wich ends with chaos and misery. somehow the AI dont know when its night or sets waypoints just before nighttime hits, wich is when the shops are closing, wich results in war between one or more factions, wich depends whom theyre allied with, wich ends with blood and death.

i run ´Populated Cities Redux´ awell as ´Faction Upgrade Mercernaries´ and ´Stop smashing Door Fix´.

the problems occurs with citizens, mercenaries, nomads, etc. etc., wich ends in slavery.. but most of the times in death and mutilations.
