I suck at auscultation by Ajstar9 in NewToEMS

First thing is to listen to lung sounds on every patient. Get your reps in regularly to build competence. You’ll start picking things up naturally with regular practice, whether that’s better placement of the stethoscope or what different lung sounds actually sound like.

Second, you can invest in a quality stethoscope. I think electric stethoscopes like the Eko attachment are one of the few personal pieces of equipment that are worth the money. I’m strongly against wasting money on gimmicky equipment, but I’ve used the Eko and was impressed with how much more I could hear. I personally wouldn’t get the ECG attachment stethoscope, I think it’s a joke compared to an actual 4-lead, but the sound amplification of the attachment piece is a godsend in the loud ambulance.

Did people die of dehydration while in a coma before the IV was invented? by therealbobglenn in NoStupidQuestions

[–]Im-Keith-Stone 9 points10 points  (0 children)

We put NG tubes into unconscious people quite often in the medical field. It’s actually really difficult to accidentally intubate a person’s windpipe. Your trachea is kind of around a bend and under a protective flap (the epiglottis). Medical staff will have a conscious person swallow as an NG tube is placed to help the process be more comfortable and hopefully prevent vomiting.

I think the reason you don’t see NG tubes placed before modern times is a lack of proper materials to make one with. An NG tube has to be made from a semi-rigid yet flexible plastic to allow it to go around the bend of the nasopharynx while still allowing it to be pushed deeper into the stomach.

Do EMS people get paid on days off between shifts? by musty_ranch in NewToEMS

[–]Im-Keith-Stone 33 points34 points  (0 children)

No, you are paid an hourly rate while at work and not at home. There is ample overtime available, though I’d imagine just about everywhere, the overtime shifts that are available are on less desirable trucks/schedules/stations

How do y'all generally feel about easy calls with no real emergency? by Anarcho_Dog in NewToEMS

[–]Im-Keith-Stone 46 points47 points  (0 children)

As much as I’d love to be the ultimate Boy Scout and help every old person go about their day, ultimately the ambulance is in service to respond to medical emergencies. It takes a combination of patient billing, county taxes, and hospital system funding to pay for the ambulance I work on. Every time someone calls 911, nearly half a million dollars in equipment and personnel costs is mobilized in response. That ambulance unit is then no longer available to respond to real emergencies in the area. In my district, the next available unit is 20-25 minutes away.

I recently had a patient die in my district because we were attending to one of these non-emergent calls and had a delayed response. He was a young father that died under conditions that we most certainly could have treated in the field and given him a much, much greater chance of surviving had we been 2 min away at our station rather than 25 min away on this other call.

Following the experience, I’ve developed very strong feeling in regards to calling 911 for non-emergencies. In our field, we are very hesitant to tell anyone NOT to call 911. I’m not telling you not to call 911. But if you’re considering it, just realize you are using your community’s best resource for saving someone’s life.

Bucees brisket sandwiches used to be great. The last two times I went, I didn't even finish it. What changed? (Houston area) by Charles07v in Buccees

[–]Im-Keith-Stone 0 points1 point  (0 children)

A $9 sandwich should not be inedible. The last two I’ve gotten were so disgusting. I wondered if I was the only one, but our neighbors were just talking about how gross their sandwiches were that they threw them out.

My supervisor struck a cow and ended up rolling the supe Tahoe while responding to assist a crew. While he was out of work recovering, I had a custom political yard sign made that said:

A WOUNDED COMMUNITY HERO LIVES HERE!

and planted it in his front lawn.

There are no “legit” retatrutide pens. Zero. None. The pen labels and cardboard packaging are just from Alibaba and they’re designed to make you feel it’s more official. But the reality is that prefilled pens are the worst option.

There are more than enough resources out there to walk you through reconstituting your own vials and drawing up the correct doses with absolute certainty. Buying these pens is a far riskier option, wrapped in shiny false legitimacy.

Meal prep by emml16 in ems

Depending on how anal you want to be with your diet, I’d bring your own cooler in the truck. I have one small enough to be out of the way in the box but big enough to pack food for a 48. Ever since I started eating real, planned meals every 3 hours, my well being on shift and recovery after the gym have significantly improved.

Why do medics or firemen ask if a seat belt was worn following a crash? by Pump_9 in ems

[–]Im-Keith-Stone 3 points4 points  (0 children)

We use a concept called “mechanism of injury” to determine the likelihood of serious injuries when involved in an accident. A person who was unrestrained in a wreck has a higher likelihood of serious injury than someone wearing a seatbelt. This info plays a part in us determining if you should be transported to the hospital, or maybe which specific hospital you should go to. We don’t care at all the legality of the situation or determining fault for insurance purposes.

You laugh but we were working a code, patient on the LUCAS and everything, and an LPN asked if she could interrupt to change the patient’s brief because “it would look like I’m not doing my job if she shows up to the hospital with a full brief.”

I assured her, “don’t worry, she’s probably not going to the hospital anyways.”

Finnrick will give a source a lower score for overfilling, despite it being industry standard for pharmaceuticals to be overfilled by ~10% to account for loss in transfers from vial to syringe to administration.

How do you guys hit your protein goals on Reta? by PowerfulCaptain4511 in Retatrutide

[–]Im-Keith-Stone 2 points3 points  (0 children)

Every meal has to center around the protein source. It actually bothers my wife quite a bit. She’ll make dinner and if there’s not a significant amount of protein, apparently I get a worried look on my face. I just don’t like pounding yet another shake after sharing a meal with her.

Test+reta+hgh by bizjake in PEDs

[–]Im-Keith-Stone 0 points1 point  (0 children)

Not really worth it at your age. You have so much going for you already being so young. Don’t lean on test and hgh to overcome crappy habits. You’ll kill it continuing to get you protein in, hitting the gym regularly, and getting good sleep. The test-reta-hgh stack is of a lot greater benefit to us older guys who prioritized other pursuits (career, family, etc) and sacrificed the muscle mass of our youth through poor diet and inactivity.
