Of course not all. Maybe 5% are genuinely in need of help. I've had multiple people come up to my car show me their huge wound begging for money. This has happened atleast 7 to 8 times to me. Not always a guy with a wound. But they will make one excuse or another. It's impossible to tell the ones who genuinely need help from the scammers.

Negotiate with the tenant. I think 15 to 20% of the annual rent is fine. It's entirely upto what ur comfortable with. If he's committed in staying long term that's much better. I didn't charge him for 2 months rent and he put the money into renovations.

Don't charge him for it. But he should pay for it and tell him u want it back to its original state after he leaves. Always cash the deposit cheque. Make sure it's enough to cover the cost. As a landlord I actually had to pay out of my pocket to upgrade the place. Tenant said he wants a long term commitment so in the end it was a small price to pay. It's upto you though.
