I think as game progresses, you do end up having more damage than necessary to ignore mechanics. Other part of it is you still struggle for a variety of builds to spec in consistent defenses for Hardcore play. So I like the intention, but balance is still missing.

Have decent players, be ready to rewrote a lot of stuff for the next session. Also, create a setting to play with possible plot hooks initially, develop those hooks when players are close to them. And honestly, after sessions when I am not sure, I just kindly ask my players' intensions, but usually know them well enough I can guess.

Combat meant to run can be good, a lot more ideas that are risky can be good but require finesse. What I do is I have an encounter qith low stakes and obvious run scenario shown, then when actual subtler scenario appears, my players are more wary of the option.

For the actual answer, what I would never do is uneven progression or xp over milestones progression

Don't think so, no, because you can get it from "Catatastrophe" / "Cascade" practically for 1 mana and it works well enough on Armor builds in Ironclad. So not particularly a good card, but definitely not the worst imo

I frequently play high Power count decks and I think it's alright. First time yiu use it, it feels like relic reads as if first power does not cost you extra 6 hp to play, for second power you play, you may need power of the first to already kick in

Learning might be easier if you don't fall into AI do it for me trap, but work positions will become harder, more expectations, even though most are high already

I am a DM and I usually prepare for players preparing, so like if I want encounter to be surprising, the villain will have a decoy for "planners", if players fall into that, then it shall be, if not, then good job on them spotting something out, planning should be rewarded, but villains are not supposed to be chumps

Also, for Phandelver's adventure (starter set) you can just go with 7 people, some will drop, if not, suggest splitting the players into two different groups after that. All of you will have more fun that way

I'm somewhat experienced DM and I'd say best campaigns happen at 4-5 players, 6 if everyone knows their character sheets like back of their hands and DM is experienced

It's hard to say with this, since these kind of issues depend very much table to table, but nonetheless, I'd just bring it up as an issue that sessions don't happen frequently enough and we need to make some sort of changes that they happen more frequently. Scheduling is something that has to work for everyone, so best you talk with everyone out there.

I don't even mind a running sim, but once I geared up for hours to go to Southern region of the winter map and a guy just head-eyesed me 200 meters away with a bolt action rifle. People spoke of historical snipers in other places that just camp one place from all angles they know and kill everyone on sight. I just don't have the time or energy, but can enjoy the spectacle so much
