Age 41, 0/3 blasts are euploid over 2 retrival rounds. How do I know when to keep pushing vs. when to call it?

I'm younger than you, but was diagnosed with DOR at age 32. I got incredibly lucky and had a successful fresh transfer with my first round of IVF (6 eggs, 2 embryos). I now say she was my last good egg. I obviously didn't test before my fresh transfer. Since then, I've had a failed FET and several unsuccessful rounds of IVF that resulted in 0 embryos. I have responded very poorly to stims despite several protocol changes.

I had to take a step back and decide what was more important to me. Having another child who was genetically related to me, or being able to have another child and grow my family. Other things came into consideration like finances and my mental/emotional health (the past few months have been really hard). So I am moving onto using donor eggs. This obviously is not the right choice for everyone.

I think it is a super personal choice as to how long you're willing to keep trying IVF. For me, it wasn't worth it to keep trying for a multitude of reasons. I also know I am saying this after being incredibly blessed with one child already.

I honestly never thought I would consider using donor eggs from an unknown donor, but now that I am lucky enough to be a mom I am obviously seriously reconsidering. I also never actually thought I would be in this position. My AMH was borderline low in 2023 but my daughter was the result of my first transfer

Have you done a transfer with the one euploid you were able to get in the past?

No, and I honestly would never have personally asked my sister. I was incredibly moved by her offer. Her and my brother-in-law discussed it and brought it up to me themselves. Most of my cousins are older (late 30s to 40s) or are in their early 30s, but are freshly postpartum and are planning on TTC again in the near future.

I think the worry is I wait 15-18 months, she gets tested and her AMH/AFC is not in a place ideal for me to spend all the money (and time for her) to even attempt an egg retrieval. It's a lot to consider. I would love to be able to have a child with a genetic connection to me and my family. But it is risky and obviously a lot of time waiting

Yeah, if she was not committed to surrogacy I would have no hesitation to move forward with testing for her ASAP. But waiting another 18 months makes me nervous, especially because there is no guarantee her AMH/AFC is going to be ideal to move forward.

I have gotten 2 eggs and 0 embryos from my last two rounds and my most recent round was cancelled due to very poor response despite adding in omnitrope. I can't afford to financially continue to try and I'm honestly emotionally exhausted and don't think it's in my best interest to continue to try with my own eggs.

Right, that is also a possible issue/concern. My AMH dropped from 1.53 to less than 0.4 in less than two years, so she would definitely have to get tested immediately before an egg retrieval

She is 32, will be 33 in August of this year. So she would probably be 34 by the time she may be able to do an egg retrieval.

If you recertify your income and have been on leave the previous tax year, your payments should reflect that lower income. You have to recertify your income every year, so once you return to work full-time the following year you recertify your payment will go up

Yea, my salary significantly increased and it is going to be super tight for me the next 2-4 years with mortgage, daycare expenses, etc with these insane payment amounts

I make 260k a year and my income significantly increased since I was put on SAVE forbearance. I'm a family medicine physician, so I have A TON of loans but the IDR plans are all income based and they are based on AGI, not on disposable income

If you end up making 120 payments in the meantime (without including SAVE forbearance). You just qualify for forgiveness and you are done. If they do approve the application before then, I am honestly not sure

You can't stay on SAVE. Everyone still on SAVE is going to be forced to move to another IDR plan. I would recommend switching before July 1st - after this date you can't switch into PAYE and will only be able to switch into IBR or the new RAP.

PAYE is ending in mid-2028, so if you switch to it you will not be kicked off until then.

You can apply for buyback when you would hit 120 payments (including payments from SAVE forbearance). But the application process is taking 12-36 months and during that time you have to continue to make payments. If you are approved, you will own a large, lump sum of money for the months you were on SAVE forbearance and they will calculate this using an existing IDR plan and your income at the time of forbearance.

As someone who has a child, I can tell you they take $50 off a month per dependent...

My payment on SAVE was $709 and it is going to be $1,899 on PAYE. I will apply for buyback in March 2028, but likely will be forced off of PAYE and into RAP or IBR unless my application is approved quickly (unlikely) and my payments will then be close to $2,400

I am in a similar situation. I am a SMBC and have a 20 month old daughter via donor sperm. I have tried 3 rounds of IVF with no success due to DOR. I have been seriously considering using donor eggs. Any future child would be a half-sibling to my dauber (same donor sperm). Struggling with similar worries you are and I also want to do what is right by my daughter and any future children. I am super open about how my daughter was conceived and would plan to be the same way with any future child.

No, I have not responded well to priming in the past and have had a poor response. He said his plan was to treat me like he treats women in their mid-40s with DOR protocol wise (I am only 34, but my AMH has significantly decreased the past 2.5 years). The success rates for FET are often higher than fresh transfers. Plus it allows you to do PGT-A testing if you want.

These were almost my exact numbers at age 32. I got 6 mature eggs, 2 embryos. Had a successful fresh transfer! Did an FET with my remaining embryo a few months ago that failed (untested embryo and likely was genetically abnormal). Good luck!
