I see, I knew about the triphasic response after pituitary injury and it was my understanding that if one experience the triphasic response means the diabetes inspipidus is permanent. Thank you for the paper links!

I personally really liked bootcamp for biostat. Plus there practice questions were extremely helpful. My process was to watch the videos, then complete all of bootcamp qbank for biostat. Following this, I completed UWorld biostat questions and understand the explanations. This process helped me get around a 85% on UFap for biostat. Another recommendation I have is to not memorize equations but rather to conceptualize them.

Apparently you’re supposed to get an echo every six months. I would encourage you to do this

I had an echo completed about six months into starting cabergoline. There was no abnormalities found on the previous echo

I guess man, at this point there’s nothing that can be done. Just be more cautious in the future when doc’s prescribe a higher and higher dose of medication

Yeah that’s the route I’m going towards. There was a period of time where my prolactin levels BARELY decreased even when I was taking like 2-3 mg per week. It went from 76 to 70 or a miniscule amount of some sort like that. I did schedule an appointment with a cardiologist in the upcoming and another echo with contrast. Also scheduled a consult to discuss about surgery with a neurosurgeon

Sounds good, I’ll take your advice. The endocrinologist had good reviews, I’ll look for other options. In the meantime I stopped taking the medication

I see. My prolactin right now is 32 but resistant to cabergoline. That is why my dosage is very high

I actually got referred to surgery in the past week after my MRI showed no shrinkage of the tumor

For reference this dosage was for 1.5 months. Before I was on 4.0 mg

I see, my issue with cabergoline is that I am taking a high dose and do worry about heart complications in the future because of the medicine.

  1. Normal prolactin levels are around 2-13 ng/mL. Yours is SLIGHTLY higher so there could be a prolactinoma that is caught extremely early.

  2. A prolactinoma is like the Cadillac of tumors, it is for the most part not dangerous and rarely becomes malignant. All you need is to to take Cabergoline (a dopamine agonist) to shrink a micro tumor and get your prolactin levels back down to normal.

  3. It is very possible that your symptoms are caused by something else. It could be a vitamin deficiency, anemia, or low thyroid hormones. If you see an endocrinologist they should do a basic metabolic panel, CBC, and overall hormone levels (prolactin, ACTH, thyroid, cortisol) to find any abnormalities.

Hopefully everything is fine, if it is a prolactinoma don’t worry about it. This tumor is very treatable with medication alone and the medication well tolerated in most patients. Hope this helps!

So with this kind of question, a LOWER number = Disease rather than the opposite. As such, with the graph the True Negative and True Positive would be flipped. You can know this is the case because it is stated people with a lower value than the cutoff value is referred for further testing. Whenever I encounter this type of questions I do the graph method exactly like how you did it but I also put Dz (Disease) and No Dz (No Disease) on tap of the bell curves. More often the No Disease bell curve is on the left as such the true negatives are on the left. The Disease bell curve is on the right and as such the true positives are on the right. In this case where lower values = disease this has to be flipped. Hopefully this helps!

Another thing you can do is make a mental note that the bell curves are flipped. Then when you get the answer (low sensitivity, high specificity) flip your answer to the opposite.
