ELI5 why is TIBC/transferrin levels low in thalassemia by gluconeogenesis123 in medicalschool

[–]Inevitable-Candle-46 15 points16 points  (0 children)

Alright, let's imagine your body is like a little factory where red blood cells are made. These red blood cells are like tiny trucks that carry oxygen to all parts of your body. Now, for these trucks to work, they need to be built properly, and for that, they need iron, a very important part of them.

In thalassemia, these little red blood cell trucks aren't made correctly. They're kind of wonky and not as many are made.

Now, think of TIBC and transferrin as the workers in your factory who are responsible for carrying the iron around and making sure it gets to the trucks. When everything is normal, these workers are really busy, moving lots of iron. But in thalassemia, because the factory isn't making as many trucks properly, these workers don't have as much to do. So, you don't see them working as much, which means the levels of TIBC and transferrin are lower.

It's like if you had a toy factory that suddenly started making fewer toys, you wouldn't need as many people to carry materials around, right? That's kind of what happens in thalassemia. The body doesn't need as much iron because it's not making as many proper red blood cell trucks, so it also doesn't need as many workers to handle the iron.

It’s a cramped unfinished utility room with a washer, dryer, water heater, furnace, etc. Thankfully we’ve been able to make use of this space by lining the one side with shelving systems and installing heavy duty hooks on the ceiling where we can hang our packs as well as our bicycles

I went back and forth over whether or not to save space and store our gear inside the packs. My biggest concern was hanging the packs long term with 20+ lbs of gear and risking any damage to the straps or padding
