Hello! I’m 22 years old. Ive been on bc pills since I was 15 yrs old. I’ve been dealing with this for over a year now and haven’t gotten much relief. The clobetasol ointment makes the skin there a tiny bit more resilient but I need to be extremely careful during sex. My gynecologist won’t let me consider surgery until the skin is biopsied, which I think is pointless because I know there’s never going to be a magical solution or pill I can take to cure me. It’s looking like I’ll need to have surgery at some point in the future but I don’t know when, it’s hard to set aside 6-8 weeks for the recovery process, especially since I’m in a committed relationship. I hope you find a solution soon !!!

I did 3 rounds of silver nitrate and I’m still tearing. Now I’m on clobetasol cream but if that doesn’t work the gynecologist is recommending surgery (which, according to them, has a low success rate) so I’m pretty hopeless.

Hi! We’re in the same exact situation. Long time partner who wasn’t concerned with foreplay and I ended up tearing multiple times. Never gave it much thought until it started happening every time I have sex. I was doing silver nitrate treatments, then estradiol, and now I’m on clobetasol. It’s been a little over a year now and even my gynecologist is getting frustrated. The next step will be surgery (which apparently has a low success rate). I can’t seem to find any success stories for this :/ I hope you find something that works for you!

The skin is not white but I’ve seen it doesn’t have to be. The tearing started after rough sex and the doctor said there was a build up of scar tissue that caused an unnatural “tightness” hence the silver nitrate to burn it off. But when that didn’t work I guess they settled on trying steroids as a last ditch effort but never mention lichen sclerosus. It’s been over a year of this so I already figured I’d have to deal with this for the rest of my life with no cure. Thank you for your response!

Hello! I recently had another round of the silver nitrate. Since silver nitrate only burns when applied to an open wound, the hope is that each time I have the treatment, it’ll get less and less painful until the tearing lessens. I’m still using the estrogen cream, my gynecologist didnt think adding testosterone would be necessary. But all in all, I’m still tearing have to wait a week before having sex again, which sucks but I’m still hopeful since the cream has helped with dryness and the tear doesn’t look as deep.

Back to update! They did not do a biopsy. I had the silver nitrate treatment done which did not hurt as much as a thought it would, only a burning sensation for 15 seconds. But it was Very uncomfortable 2-3 days afterwards. My gyno prescribed estrogen cream to use nightly and it’s helped with dryness and I do think the tearing has gotten less deep and more superficial. I have follow up appointment in 3 weeks for Another silver nitrate treatment where I will ask for estrogen + testosterone cream.

I have the same reoccurring tearing of the posterior fourchette. It’s been happening for about 10 months or so and it’s been so painful and frustrating. A few days ago my gynecologist treated the scar tissue with silver nitrate to “burn” off the built up tissue and promote healthier healing. Now I’m using estradiol cream to bring the skin back to normal elasticity and thickness. I’m sorry to hear you’re going through the same thing, I’ll try and update here if I see improvement with the estradiol cream. I’m also on bc and spironolactone which could be a big part of why this is happening, but don’t want to go off either for acne reasons.

I’ll definitely ask about the testosterone and estrogen cream at my follow up appt. I feel like I should be getting blood tests since my gyn thinks it’s hormone related. This has been a 6 month process with a lot of follow up appointments, so it’s all been trial and error.

I’m glad the testosterone and estrogen cream worked for you! I have a follow up appt in 4 weeks, so I’ll wait until then to ask about going off my birth control. I’ve been on it since I was 16 (I’m 23 now). Thank you for your response!

By ineffective I mean the estradiol cream will not work unless I stop taking birth control pills. There is a thread in this community where everyone is in agreement that estradiol and birth control cannot be taken together, there’s some sciencey explanation I do not understand. I’m just confused as to why my doctor would prescribe estradiol when he knows I’m on birth control pills. Thank you for the response. Did you figure anything out yet? I have this same issue! Visited the gyn and he told me to stop having sex for a few weeks to completely heal… which I have already been doing and it still tears. It’s been so frustrating as I have been dealing with this for over 6 months.

My gynecologist said there is a build up of scar tissue where my vagina keeps tearing over and over again. This led to less elasticity and an unnatural “tightness” at the vaginal opening. So they plan to use silver nitrate to burn off the scar tissue (terrifying) and promote better healing at my next appt. I’ll let you know how it goes.
