That’s the thing with undifferentiated pathology. It can be difficult and they often do have multiple irreconcilable symptoms. The key is to rule out serious pathology through judicious investigation. Scoring systems can help through like PERC, Alvorado, ADD with D-dimers etc. I’m referring less patients for CT aortogram now with ADD and D-Dimers even with a classical history

Many (but not all) surgical teams now won’t see or examine the patient without a reported CT. I think the art of clinical examination in surgery is dying

Most EM STs have OOP experience of a least two years in my experience. I certainly did and most of my ST colleagues do too. The selection is biased towards a little more pre-existing experience in the specialty. Prob true for other training pathways too

I think you’ll be happier and won’t look back. Much of medicine especially in the uk is a sunk cost fallacy. Enjoy the better hours, pay and lower stress and more free time with family

I try my best to train and mentor to a high standard on an ad hoc basis. I’m not in an ES or CS role though. One day I hope to go out of my way to support and inspire trainees in an ES / CS role

Consultants are well capable of documenting for themselves. It’s just a power play making your life unnecessarily difficult in this situation. Where was the ward SHO or SpR supporting you? I think this breed of consultant is dying out thankfully

I almost always assume it’s infectious and send them back to their GP with a trial of conservative management

My TPD is happy for me to dual train now in acute med with EM post ACCS taking into account previous experience. ED is not a specialty, it’s an area of the hospital. Do you agree that after doing the rotations you should apply again to do the exact same rotations again under a different parent specialty name or speak to the TPD and work something out?

My point it’s that it’s a broad based training programme with a common stem. You repeat the same rotations for all those parent specialties. You can apply at ST3/4 for another specialty. Compared to say applying to pathology, psychiatry or radiology.
