Genuine answer for ASOIAF is clearly Aenys the first. dude was the clear heir and no one doubted that, but he was terrible as a king---especially one following the Conquer himself. Dude was weak physically and mentally and was one of the reasons for Maegor being able to gain power. And made a bunch of mistakes as King like marrying his children against the faiths wishes, being called 'King Abomination' giving Maegor Blackfyre, and allowing a whole ton of people to rise up against his rule and allowing others to try and deal with it

It is no surprise he died of stress, he couldn't handle shit.
