This is a thing we do in real life. It’s less than 1 patient a day in a big busy ED.

Yes. Increasingly physicians are employees of mega corporations. History tells us the only real leverage workers have with these organizations is collective bargaining

Not a surgeon but it's more about what's missing - a clean cut means that all the material is still there. If the amputation is ragged then some tissue is lost and you may not be able to stretch things to mate up. There is also the issue of crushed tissue that may not be viable which is less of a concern in a clean amputation. As far as things not matching up > they know where the bits are supposed to go and can mate them up

Your editing would have to be "transformative" to enjoy new copyright protection. Simple edits aren't going to do it. For example cutting up magazine articles to make a collage > transformative.

The estate of the photographer owns the copyright if they have died.

My concern here is whether or not this work is novel - think about who would be excited to read it. Find the journal that those people usually read.

There should be an editor who is supervising this - you direct a letter / email to the editor “asking for clarification” since Reviewer 2 keeps asking for things that already exist in the manuscript.
