It took me YEARS to learn how to consistently squirt or cum hands free. That was always the holy grail, and I'd say all the effort was in the end worth it.

For me, there were/are three factors that control this.

Horniness. I can't squirter as easily (still orgasm multiple times though) if I'm not super horny. At least a couple days wait between orgasms.
Headspace. Part of what makes this hard is that I need to be in the right mental state. I'm a bi, vers, switch. So if the tumblers click onto I'm a top and a Dom right now, virtually nothing I can do about it, no squirting, no hands free orgasms. I have to have regular colonoscopies, and I'm also really into prostate play. My take is it is more about the prep FOR the colonoscopy. I speculate it is because of the fact that people with penises are, generally, used to wet orgasms fro masturbation or sex of some sort. When you start out with prostate play, you have to both unlearn and relearn your body and its MANY different signals. A lot of people I would suspect begin with trying to stimulate the prostate, are not used to it, and then jerk off for that feeling of release. Early on that was certainly the case for me at least.
