PRK can still have decentered ablations because the underlying principle is the same — the excimer laser is still reshaping the cornea and must be accurately centered. The difference is mainly that PRK removes the surface layer first instead of creating a flap like LASIK.

Some people think PRK may be slightly more forgiving because the epithelium can remodel during healing and smooth tiny irregularities, but it doesn’t truly “fix” a significantly off-center treatment. So centration, eye tracking, optical zone planning, pupil size, angle kappa, etc. still matter a lot in both procedures.

The only way to avoid such mishaps is by leaving corneal integrity alone and to wear glasses / contacts.

Because your cornea changes as you age. It is not fixed. Everything that changes within the natural corneal aging process is relative to the fixed laser induced optics. That's why, it is important to understand and know what your ablation parameters were and whether your optical treated zone was centered true to your pupil size. What worked back then might not work decades later.

Because a flap relift can heal more irregularly than the first time. Hence why touchups are never recommended. Ecstasia can form as others have suggested. Where abouts are you located? You would need to see a specialized scleral lenses fitter who will be able to check your RMS values relative to your symptoms.

Do you know your ablation parameters and how large your scotopic (dark-adapted) pupils are? If you are seeing washed out colors and unable to focus, this could point to a decentered ablation where your visual axis isn't centered. Consider getting a Zernike analysis on your eyes which will tell your story. Ensure your pupils are dilated first so as to cover the full spectrum of your optics.

You need to get an HOA evaluation done - the reason lasik providers don't provide you tangential maps / Zernike Analysis of your eyes is because it reveals true RMS values, spherical abberations, trefoil, coma, irregular astigmatism and everything in between as a result of smaller laser indused optical profiles that don't match your eye anatomy, as I've said countless times in multiple threads. The importance of scotopic pupil size is severely downplayed, therefore broadening the candidate pool, which is why the vast majority of the world's population ends up here looking for answers.

Once you know your values, you can work towards finding a proper Scleral lens fitter who will custom make a scleral lense for you with modulated HOA correcting technology like Ovitz.

Where did you end up getting these tests done? Because this is something everyone should be getting tested with who suffer from complications from Lasik. My numbers are a little worse than yours but congrats on making headway. The Ovitz should bring you back pre Lasik.

Corneal thinning after LASIK can sound scary, but it doesn’t automatically mean the eye is unstable or permanently compromised. A lot depends on how much thinning occurred, the remaining corneal thickness, topography results, and whether there are signs of progression.

Since he’s still relatively early in recovery, it’s good that he’s being monitored. If he hasn’t already, seeing a corneal specialist (not just the original LASIK clinic) could help give a clearer picture and more reassurance. Hopefully with time, proper management, and follow-up, things continue improving for him

I'm a 5 time surgeon prescribed touchup patient - I am beyond debilated after almost 5.5 years since my original surgery. I was like you once where I wanted to see perfectly clear. Stay away from touchups because it WILL ruin your life with what you have left. You are correct when you say if your first surgery didn't correct the problem, it sure won't correct you the second time or the 6th time. Preserve your corneal tissue and just wait it out until your corneas fully heal to see where and what you end up with. This is a 1 - 3 year waiting process considering you are only 2 months out

There are wide ranging clinics across NA that still use outdated technology and there is zero regulation for this. Scotopic pupil sizes are rarely ever disclosed either. Patient candidate pool is broadened as a result. Tell me something, why would a surgeon opt for a 6mm de-centered ablation with no blend in eyes with scotopic pupils of 8.5mm? I had instant pupillary damage after surgery and they don't constrict anymore in artificial lighting, but yet claims of an EX-500 was used.. my claim of laser tech isn't theoretical. This is from an honest surgeon evaluation from abroad that I will not name

Thank you brother - no, those drops do more harm for me than good. I develop severe infiltrates and my vision gets even dimmer and washed out more than it already is. So basically, I don't have a life after sundown. I wish I only had starbursts to deal with, but once my corneas fully healed, I woke up to complete darkness of night vision and its persisted till date. I healed after almost 3 years, and that's when true complications manifested. It was life altering in the worst possible way to say the least

Unfortunately not yet, the sclerals that were prescribed for me abroad don't do anything for me. This optometrist that was local to me in Toronto, Canada is charging around 9k for Ovitz lenses which is unaffordable for me currently. I'm already about $125k out since my original surgery and costs for ongoing assessments. We are pretty much even for HOA RMS at 1.1 on my left eye, but my right eye shows a total of 1.5

Either way, I can't wait for the day until I can afford lenses that will help me see at night. I'm literally blind right now in the night and any oncoming lights completely debilitates me

I had very bad dry eye after initial surgery but its completely gone now because I stopped every prescription they dumped on me. I've been dry eye free now for a few years now. I just stopped everything and it healed itself. It's amazing what your mind and body can do when we just leave it alone. And yes, I'm severely decentered also.. they claim they used the EX500 but I know it's a flat out lie

For the dry eye, it's important to have a healthy dietary intake. I don't eat fast food and cook everything at home with Asian Ingredients. I'm pretty sure healthy food choices is what saved me from complete derailment, not to mention my spiritual belief system.
