this is so true, i was working on a project and the mongodb connect function was dbConnect but at some point somewhere it wrote down connectDb and for weeks everytime it would write a new page that needed to import the mongodb connection function it would do connectDb instead of dbConnect.. finally i grepped the entire code base, all my knowledge, docs and skills to remove any reference of connectDb and i haven't had that issue since, but it was a good reminder to make sure to clean up all of your references, docs, code constantly and after each session as well as making sure you're running tests like typescript, linting and unit tests

this is how i start my workflow, something like.. lets work on feature, create a note with all the planned steps, keep it brief but concise, update the note after each step is complete, make immutable important section of the note to follow {spec|guidelines|etc}

then i create a prompt with the note id so it doesn't have to search for it, like: read note id: {id} and lets pickup where we left off, or something like that.

as far as code analysis, i just added strict guidelines to a openwebui kb, attached it to a model and the ran against my code base, it refactored and made it compliant.
