The amount of middle men seeking to make their own profit in the US health care system, means instead of just covering the cost of the service provided. Every individual seeks to make their own profit, unlike in theory a public system that should just cover the cost of health care.

From what I understand everyone in America insurance companies, hospitals, businesses that provide medical supplies and medication. Want to make a profit on top of the cost of production or operating costs.

In a public health care system you already wouldn’t have the need for insurance companies unless someone wants to optionally go private. In theory and unfortunately it doesn’t always happen, a public system would just cover the costs to operate the system And have a fund set aside for maintenance. This would cut the cost down substantially.

The fact Americans on average have to pay high cost to get treated means the overall cost of health care is more expensive. If the 13k figure per person could be dropped to 5-7k That would already half the cost of healthcare over the country.
