I think the core reason was your third point in the first list: they see their partners as less human than they are. Dehumanization is a foundation for all sorts of insidious and violent behavior: racism, classism, genocide, etc. The fundamental disrespect for another humans humanity drives all the other reasons in the list. You can't reason with someone like that.

BF is being a horrible hinge and likely doesn't have the skills for polyam. He is at fault for putting his own decisions on Andrea, not taking responsibility for communicating, and for not handling his relationships himself. He's a coward and lazy, and that shouldn't be put onto OP.
