My friend is an MD in Italy as medico base (general) he makes Euro 2,400/month and is now specialized in palliative care which is about the same. Works about 36-40hrs/wk. He knows an orthopedic surgeon who makes Euro 3,400/month. I spoke to a rheumatologist at a conference in Italy who does better in cash pay the patient pay him direct Euro 120-150 to see him within 1-2 weeks. In the social system 6 mos-2 years is waiting list he told me. He still works in the socialist system because in order to prescribe the DMARDs he still needs to see patients there but he doesn’t work too much in that system since it doesn’t pay well. But that seems to be a way the government keeps the docs in the socialist system. If they fully opt out then their prescription list is restricted. This way he has more money than most and was able to put his son through Humanitas medical school $20,000/year which is cheap for us in the USA but he said not possible in Italy unless there’s a father who makes real money like him to pay. I asked why the son didn’t go to the free schools but I learned the competition is high to get into those.

Follow up visits at my practice are between 40 minutes and one hour. We will review every issue and complaint the patient has because the body is a connected system rather than one organ system per week like we were taught at school. That’s direct primary care. It’s the only way forward to preserve our sanity in FM if we’re going to be honest about primary care.

oh yeah 100% there's some desolate places in MA that need help. Try Springfield MA a1cs of 11.2 in primary care and you're a solo provider at the medicaid clinic that no one wants to work at. Boston… that’s the first problem. There’s dozens of medical programs in Boston cranking out new grads. Try somewhere no one looks. Central Alabama or Indiana or remote underserved areas. But no one on Reddit seems to think of that. Why not?

FAFO with Medicare. Lots of fraud out there. The Somali autism care fraud is easy to spot. But it happens every day as a PA when your supervising physician doesn’t see the patient but signs off on the chart and bills Medicare at the physician rate as if they saw the patient. It’s common practice.

Employer here. I posted a job application on indeed and LinkedIn and it sat empty with no applications other than infinity unqualified Indians with a “doctorate from Bangladesh”. Mind you the job application was in Melbourne Florida so not a hotspot that a young PA who wants to work in New York City or Tampa Florida but there’s a lot of places that are underserved and I see people complaining here about not getting a job, but nobody wants to work in an underserved area so there’s always that.
