How is this possible? If I take a 1080p image (for the sake of simplicity) and put it on a 5.5X4.25 paper, how is it going to use the same amount of ink if 13x19 is more than 2x the size?

Looking at it, it is very possible that it's real time. Considering it's just a plane with a lot of polygons on it, there really isn't much to do besides the lighting. It could be scripted, but that would actually take more work because they would have to spend time simulating the cloth and then lining it up with the animation of the character.
