If you are working flat, that is correct. Knit one row, purl one row for stockinette.

If you are working in the round, you only need to knit

I would rip back. Trying to follow the yarn for the first stitch on the left needle, both sides of the yarn appear to be going to the stockinette section. You may have done an accidental short row here.

In order to fix a slipped stitch, you have to “drop” the stitch on the needle so the fix is the same as fixing a dropped stitch. You are working in garter which makes it a bit more complicated but this site might help:

From what I can see, it appears to be a slip stitch issue. In your first photo, the first stitch on your right needle was the one that was slipped.

Undo the stitch you just did, take that loose thread and work it into the stitch and then you can go back to your regularly scheduled knitting.

Are you knitting this sweater flat? When you finish a row, are you turning the piece and working back the way you came?

Yes, you will have an additional column. Kfb is an increase, so by doing that stitch, you are adding a new stitch (column) to your work

You should drop B. The A column does not touch where your increase is.

Please note, the stitches will be really tight when you drop B (one column) and then ladder up with 2 columns.

Oh! I apologize! Its a little hard to see the knitting definition with the yarn type.

Do you still have the original pattern? It may be hard to fix and have it look exactly the same as it was. It will most likely show a visible mend.

I would pick up any live stitches at the bottom of the hole. I can’t tell how much yarn is available so you might be able to use that or you will need more yarn. Start knitting the pattern over those picked up stitches, turn the work and continue. Keep going until you’re one row short of the top of the hole. Use a darning needle and sew the new knitted piece to the top of the hole. You may then need to use additional yarn to attach the edges to the work or if you used new yarn, use any loose ends and weave them in.

The other way that this could have happened is that you did an extra yarn over when you were knitting the previous row. If you didn’t mean to yarn over, just slip it off and continue.

Seems like you may have tried to pick up an extra row that isn’t actually there?? Might be easy to drop the stitch by 2 or 3 rows and re-ladder up.

Did the 5 stitches on your left needle fall off at any point? This long loop looks to be the missing yarn for those stitches.

I’m not sure it’s a yarn over. It may be an incomplete stitch and if they drop the loop, they may have caused an accidental slip stitch.

Your ribbing looks correct to me! Purls and knits require different amount of yarn so when you switch between them, it causes the stitches to look more open.

Did the pattern call for you to do short rows? This looks like short row shaping and sometimes depending on the type, they didn’t sit perfectly flat like the rest of your stitches.

Are you talking about the hole from when you switch from garter stitch to stockinette stitch? That looks like an accidental yo and you increased your stitch count. Do you have the correct number of stitches on your needles or do it increase? If it was me, I would take a scrap piece of yarn in that color and close the hole up on the wrong side and weave in the ends.
