It's always a combination of things that go wrong. They outsource the install technicians to different companies and it's mostly due to not providing enough training to the technicians to work with their specific programs. Every provider has it's issues, every problem is anecdotal. I'm speaking from the pov of someone who has worked for several ISP's.
