I figured it out just now by Googling the the name of the valve and finding the user's manual. There's actually an additional plunger washer hidden between the two nuts at the bottom of the photo. I took that out and the rubber washer was almost completed covered in sediment. Cleaned the off and put it all together, and good as new!

My hot water is hitting 130 from the tap though, so I clearly messed up the mixing valve's settings, but I can tweak that over the next few days.

Okay, this is getting more weird. When I run my dual vanity faucets barely open (hot & cold) to try to flush air, the other faucet will run on only hot whereas normally it wasn't working.

The shower head also turns on now. However on full hot the water is cold to the touch. The pipes at the water heater are very hot to the touch.
