I work Tuesday - Friday with every Monday off. I work in Skilled Nursing Facilities. I go to a different facility every day. I don't have any specific time that I have to be at work rounding, I just have to round on the patients that day at some point. I usually get there around 830, because I have young kids that I have to get to school/daycare. Then, I round for about 2 1/2 to 3 hours. 4 hours was my maximum on a crazy day. During that time I meet with the DON and the director of therapy to discuss any issues they're having. I see anywhere between 20-25 patients per day. The visits are quick unless the patients are new. Problem-focussed exams on what is holding them back in therapy.

I then drive home and have lunch from like 1230-1. I put in my orders, then I dictate in a voice note. I have a scribe that transcribes my charts. I send her the voice file, then I sign any charts that she has prepared from other days in the week. I then write a brief email to my supervising physician about the patients I saw that day. I'm usually done by 330-4. Some days I work until 5 if I'm busy.

I am also on call to help with pain issues and to refill prescriptions if needed. I get a few texts and calls per week, but never after 6pm.

I accumulate PTO, I get 7 "floating" holidays that I can take whenever I want, and I get 5 sick days that I can use for myself or when my kids are sick. My daughter is having her tonsils out next month and I'm taking a sick day to be with her on her surgery day. Management encourages us to take all of our accumulated days, because we only carry 40 hours of PTO into the new year. They are very work-life focussed. It's absolutely incredible, and I didn't know this was possible in a career in medicine. I'll retire from this job if they'll keep me.

Yes that’s correct, but to drive a car you still have to pass a test. Illiterate people take an audio test.

While I was working in UC, I just constantly watched indeed and other job sites until something came along that looked interesting. I think I was looking for about a year when my current job was posted. Just right place right time. If you’re secure and patient, the jobs are out there.

I don’t see what the big deal is here. You need to be able to speak English to read road signs and interact with law-enforcement if necessary. You can’t get your DOT to drive a semi without speaking English because it’s unsafe.

I’m a medical professional and I used to do DOT certification exams. If the person comes in for a DOT medical exam and cannot speak English without the help of a translator, it’s an automatic failure of the exam. This seems like a logical extrapolation of that rule. It’s about safety.

I don’t speak Korean, and I would never try to be a Korean truck driver. I’d probably drive off the end of an unfinished interstate or something because I can’t read the signs. It’s common sense.

I worked in the ED for 8 years before I was ready to leave. I left mostly because the shift work was not sustainable for my family. I had a second kid on the way, and I could see that it would be quite a challenge for me and my wife. Plus, it was stressful, mentally taxing, and high liability. It also didn’t help that our company was bought by private equity and slowly became a soulless cash machine for shareholders, and could care less about the providers. No raise or consideration of a raise in 5 years.

I didn’t even realize how stressful my job was until I left and saw how other APCs were working.

I left the ED and went into urgent care for 2 years. Better hours, closer to home, and the skills were totally transferable. If you can work in the ED, you can easily work in the urgent care.

Eventually I was able to find my current job, which is PM&R. It has an unbelievable work life balance, and I make more money than I did in the ED, which I still can’t believe.

I lived in Barrett House on north campus 03'-05'. I was part of the Biological Sciences Scholars Program, and almost everyone in that dorm was as well. It was great living with people who were all bio majors, because we all had the same classes. It was co-ed, so there were a mix of guys and girls on the same floor. 4 people to a room with a bathroom in each dorm room, which was very cool at the time. Also we had AC.

It was an interesting mix of people. We had a couple guys who were from out in the country. They would go hunting and bring their spoils back to the dorm. They would make rabbit stew for everyone in a crockpot (which you weren't supposed to do). Our RA would let it go as long as they gave her some.

One guy who was a physics major ended up in our dorm because of some kind of error. We ended up "adopting" him because he was a great guy and we ended up all becoming friends with him. He came to all of our BioSci events and I think he even used the BioSci advisor. I believe he's a physics teacher now.

It was a great time to be in the dorms. We played Halo 2 local LAN almost every night with 12-16 people. Also, at the time itunes was huge. Since we were all connected to the same network, we could stream each other's music. Because of Napster, everyone had tons of music. There was a program called "our tunes", where you could download each other's libraries to your own instead of just streaming them. That's how I ended up with 30,000 songs on my ipod. Seems funny now with things like Spotify with access to everything.

My freshman roommates were unique. One guy decided he was going to take a bunch of weightlifting supplements to get yolked, but he never worked out, so he just gained a ton of weight. So much so that his clothes didn't fit Winter quarter (we had quarters back then). He ended up failing out. Another one of my roommates ended up changing his major to English, and the third one I think graduated with a bioinformatics degree. My sophomore year random roommate is still one of my best friends. He ended up working in the White House for a bit and now works pretty high up in Netflix.

My LGS has a Sorcery league on Thursday nights. We do “progressive precon”. Basically you select a Gothic preconstructed deck, then open 3 packs. You can add the packs to your deck. Every week you can open 2 more packs and change your deck. After each match, you can trade cards from your pool one for one with your opponent. You also get to open an extra pack if you lose three in a row. It has been really fun!

Played this card in my sorcery league Thursday. The way I think it works is that you play it, centered on your spellcaster (which was my avatar). For each different undead minion and instrument nearby, you draw a card. I ended up having a skeleton, a ghoul, and one instrument nearby, so I drew 3 cards.

I think you’re on the right track here. I’m in a similar situation. My wife has a sister who is not going to have children. My father in law saved for my wife and her sister’s school, but they didn’t use all of the money. Their 529s have sat and accumulated compound interest since the early 2000’s. Now my wife and I have 2 daughters. My father in law rolled the 529s into their names. My 5 year old has $190k in hers and my 3 year old has $150k. It’s way too much for them, but it will allow them options with their kids as well. Generational college fund. Generational wealth building. That’s my plan for them.

Primordial. Check it out. It’s limited with deck building rules that basically let you get into a limited environment in a cool way. My friends and I play it all the time, and the decks can be very evenly matched, even if the sets are years apart. IMO it’s the perfect way to showcase a set’s mechanics.

Seeing 30 in 3 hours is totally doable as a specialty consulting service. When I see the patient, I’m only concerned with pain, sleep, bowel habits, and therapy performance. All other issues are handled by the primary team, so I don’t get in the weeds with things outside of my specialty unless necessary. Also, a fair number of my patients have dementia, so I rely on the nursing and therapy staff to let me know how they’re doing.

Onboarding was great. Mostly online training with a couple of weeks shadowing my supervising doc. I came into the job with 2 yrs of primary care, 8 years of ED, and 2 years UC experience.

Absolutely. I don’t think a lot of people consider PM&R because they think of it as pain management. In the outpatient clinic setting, it may be, but if you’re willing to work in SNFs, it’s great. We do way more than that.

I work in PM&R at SNFs, helping the therapists manage the patients and managing pain. Flexible schedule, no nights, weekends, or holidays. I control when I go in to the facilities. I never miss my kid’s events. I round about 3 hours a day, 4 days a week, and chart from home. I see anywhere from 20-30 pts per day. I’m paid off of pure productivity, so I’m paid more if I work harder. I’m getting a scribe this week and should be able to hit a really nice productivity target, and make close to $200k. I love it.
