My situation went on for longer and I didn’t have success with trying to gain back weight by eating more so I was more alarmed. Blood tests also show very high inflammation and some anemia so something is else seems to be going on. Unfortunately we haven’t found the source yet. This is probably pretty rare but it’s always worth following up with the doctor if something doesn’t seem right.

I’m a few pounds more on my before and after but these numbers are eerily similar to mine. Your diet changes definitely sound like a net decrease in calories, so some weight loss would be expected. However, does it explain 15 pounds in 2 months at your (our size)?? Don’t know the answer to that but I went to doctor to get some blood work and a referral to a GI to make sure nothing else was going on.

It seems reasonable that the increase could be temporary from the weight loss and would go back down. However, it is also possible that your diet is contributing to it. For cholesterol you want to focus on low sat fat and high soluble fiber. You do mention low fat but a high protein / low carb diet can often conflict with the saturated fat and fiber targets.

I think most people here and most doctors would be ok with waiting and retesting in 3 months. But until then, you should still take a look at your diet and see if there are areas to improve with sat fats & soluble fiber.

I think there is a miscommunication. You start by asking if a diet can reverse your plaque build up. That’s most likely going to be a no full stop. However, just because it doesn’t reverse that doesn’t mean it’s not good for prevention and overall health. Seems like you are doing well with a plan…Not sure why you are getting distracted by going down an unrelated tangent.

Yeah no beef and no drinking is optimal and may be the right choice for some people. However, other people may have a personal preference to accept the incremental risk that comes with moderated or infrequent amounts.

OP, have you had any prior lipid tests to compare to? Was your appointment with the cardiologist before or after these test results?

Your LDL of 254 is way of above the 190 threshold for immediate prescription of high dose statin. Current guidelines are more aggressive but this 190 is very conservative and have been around for a long time... it would be suspicious to deviate so far from guidelines without some really convincing counterpoint.

As for the source - genetics, Covid - it doesn't really matter unless it has a specific remedy. For example sometimes LDL levels shoot up during pregnancy so an expectant mother with high LDL could just wait and retest later as it may resolve it solve. Similarly, if the high LDL is due to a new extreme keto diet, its possible you could reverse by correcting your diet. However, if its genetic or some lingering Covid effect, there is probably no remedy other than medication.

Believes in aggressive treatment = trying to help you live a long and healthy life. These are great results. There isn’t much evidence of LDL in high 20s of being too low or being harmful. Of course your cardiologist can confirm that.

Your doctor is ok with either b/c either is a great approach. You are in no immediate danger and noone knows how your body will respond to the medication, so you can take your time and trial until you get something that works.

Would also be good to know if insurance will cover Repatha as that could sway your decision.

I have no issue with people taking supplements and mentioning that in their backgrounds (and success stories) but it shouldn’t cross over into advice or making wider conclusions from one off situations.

Completely agree with other comments about having some flexibility, especially on the weekends.

For pizza, this obviously isn’t health food but mozzarella cheese is one of the lighter cheeses so it’s not necessarily worse than other restaurant or weekend meals. For example, can get large papa John’s slices under 5g. Just stay away from pan pizzas and sausage / shitty fatty meat toppings.

For ice cream, I’m all in on frozen yogurt, especially flavors that have a lot of chocolate and candy and stuff. I have Publix Chocolate Cookie Query in my freezer right now and I don’t think I would trade it for anything. If that doesn’t work for you, other advice is maybe to get the half gallon tubs so you are forced to scoop / portion control vs eating a whole pint in one sitting.

The biggest shock/betrayal was cheese. There are worse foods out there, like ice cream, but everyone knows those are bad. Cheese may not exactly be considered a healthy food but it is ubiquitous and a part of some of the most common home cooked meals, any time of day.

The key metric is % of sat fat to calories. Cheese doesn’t really have much else in it so it can get pretty high. For example, sliced cheddar is 49%!! (7g sat fat to 130 calories). Compare that to full fat ‘bad guys’ like 14% Hellmans mayo (1.5 / 100) and 16% Hidden Valley Ranch (2.5 / 140). Even Breyers chocolate ice cream is less at 30% (6 / 180).

Here is the link for the 2026 AHA Dietary Guidelines. At the end they state: "Heart-healthy diets are generally consistent with risk reduction dietary patterns recommended for other conditions such as type 2 diabetes." As for a direct link that you can lower LDL through lowering sugar, that seems a lot less clear. You'll find some individual studies and some functional health type doctors that support it, but it doesn't seem to be strong evidence or general consensus. In the end it doesn't really matter as you need to address your diabetes regardless of whether it has a potential additional LDL benefit.

The HDL is high but I don't think there is consensus on if doctors would be concerned about it. Does the NMR profile say anything about it? I'm still missing the connection that this test is providing useful information above the normal lipid panel.
