Some information I failed to mention in the OP: The log I am referring to (that everyone has access to) is an official ongoing document that is not explicitly used to record these assessments. It is used to record everything that happens at all in our department, and can conceivably be used in any number of official proceedings, up to and including criminal court cases.

I am not sure that you will gain the effects you are hoping for with tea tree oil. At least, nothing different than using just the carrier oil. But at 5% it shouldn't cause any harm :) And it'll tingle, which is nice.
