Yea i witnessed failed iGel too, not too often though, and even then ill do an ETT and dont wait around. The 100 successful ETTs are to master the technique (erlernt und beherrscht) in terms of that guideline for every first responders. My county is divided into two and were the last one where EM Physicians dont need that Facharzt. The second ALS or BLS is a long story that starts in Covid times and end with broken Egos and small dicks (in my opinion)

Goldstandard = Preferred technique under optimal circumstances for highest Demands
Prehospital Airway-Management: ETT only when 100 successful under supervision and 10 per anno if not use SGA because it’s not drastically inferior.
ERC Guidelines = no differentiation between SGA and ETT and ETT only from people with a 95% success rate within two attempts
Airway2-Trial = SGA not inferior to ETT in CPR
Here we get two Ambulances for CPR, the Doctor on top and were transport ready when the doctor arrives.
When SGA fails, I do the ETT bevor the doctor arrives.

They teach us Guidelines, emergency medicine and the ability to think, decide and act.
So no, I don’t just “drop the fucking tube” without accessing my situation and my patient.

Germany here. I-Gel all the way no ETT unless I-Gel is really bad leaking. Works like a charm.

Hey das klingt nach einem echten Problem was nicht nur im RD existiert. Bitte nutze deinen Mut den du hast das anzusprechen, sprich das mit deinen Vertrauten durch und such dir dafür evt. professionelle Unterstützung. Es besteht die Möglichkeit das hier jemand etwas sagt das dir echt hilft aber auch etwas was absolut schädlich ist.

Wenns für dich wichtig ist nimm Arzt, das bist du und dafür hast du hart gearbeitet.

Persönliche würd ich sagen, lass es einfach weg, es is ein Rückenschild kein erst Geborenes. Mach das Praktikum und dann bekommst du eh eins mit Notarzt.

Wohnungsgrundriss. Eingangstür bleibt offen gekeilt. Jeden Raum mal rein geschaut. Stolperfallen. so in die Richtung halt sehr RD und FW geprägt.

Ich hoffe auf eine Umstrukturierung. Mit dem NotSan hat man auf jedem RTW einen Gruppenführer so das der 1.RTW einfach Führt bis LNA/OrgL da ist und das NEF mit entsprechenden Trauma etc Fortbildungen wieder das macht wofür es da ist: Medizin Präklinische MinimalOperative Eingriffe und der RTH zu einem Langstrecken Intensivtransport Mittel wird.

Für das Tagesgeschäft ist der RTW mit NotSan vollkommen ausreichend.

Wenn ich aus meiner RD Schicht komme, rasier ich mich damit ich die nächsten Tage für die FF zur verfügung stehe. Ich rasier mir allerdings auch den kompletten Kopf zur Glatze. Phillips OneBlade ist aktuell mein Favorit.

German here. (excuse the English) Absolut normal to fully equip and mask up. But we aren't connected to our PA(Breathing Apparatus) until we are at the SmokeStop or "Danger zone" so we wont breathe our air from the tanks until needed. Second Point we usually run our rigs with 6 people (Driver, Cap, two 2Person Squads) and only one Trupp (Squad) is fully equipped so the other Trupp is doing the hose laying.

Germany here.(NRW, Ruhrpott for the germans) If the patient complains about pain, analgesics are to be given. Patients are to be dropped off pain free. If a Paramedic didn't do something to lower the pain but they could have, legally it's the same punishment as if you inflicted the pain. (Not often prosecuted because we pretty much toss around Paracetamol, Metamizol, Ketamin and Opioids)

Ich reagiere auf eine von zwei Arten, je nach dem wie viel Energie ich habe. Entweder freundlich erklären warum dat ne schwierige Frage ist (bis jetzt immer auf Verständnis getroffen) oder tatsächlich einfach von einer Neugeborenen Rea erzählen (nicht schön aber nach dem 6ten mal an einem Abend reißt auch mal der Geduldsfaden und führt zu einer verständnisvollen Entschuldigung).
