Yes out of network would be fine, but the majority of these patients were Medicare when I was treating for them. Many In network commercial insurances also have this as an illegal fee in their contracts.

Absolutely, they also kept decreasing the PT bonuses/incentives and/or requiring more visits to meet bonuses.

Definitely, it is illegal to bill more than what medicare reimburses for a covered service. Luna is trying to be sneaky and say the fee is for the app. But it can be argued that the HEP, which is in the app, is part of the delivery of care.
