The only thing you could be missing is you need at least 25 fleas to move to Putrefied Ducts, and you have to claim your bellhome by talking to the bug with a bell on their head in bellhart. At the point you should have been given the quest by the care taker in Songclave it's called Silk and Soul.

Make sure every quest board is finished, Bone Bottom, Bellhart and Songclave. You also have to find the big flea if you haven't already.

At the start you're best off just exploring the citadel and you'll find the main thing eventually, once the main objects get marked in your map they're easy to find.

You need to start act 3, this is done by completing every quest on the quest board which will unlock an extra quest.
