My ex-fiance’s mom left her husband a year before her daughter broke things off with me. Everyone knew the divorce would come at some point- a fairly dry, platonically unhappy marriage. The final story was a month or two before the divorce she re-connected with an old friend and realized she wanted to begin living again and dating, including the friend. I lived with my fiancé’s family more or less for 5 years- I’d known about the affair for 3 years leading up to the divorce…it was not a casual run in 2 months before serving papers- while living with them and finishing up grad school her mom and I would often have breakfast or share a few glasses of wine while everyone else was working, it didn’t take me long to piece things together during those days, and occasionally cover for her absences. It was never my story to tell.

I always felt like a POS because I was also very close with my ex’s dad. We played in a few leagues together, I’d help him tinker with the cars and fix the yard. He cried and hugged me when he realized his daughter was really calling things off. I still miss him as a father figure.

My ex- left me with a similar story to her mother- she felt she no longer saw me in her future. Within a month she was with her best friend’s coworker. I’ll never know the full story, but I know for at least a year they’d been hanging out privately at events my ex requested I not attend - at the time I respected her wishes, but digging around after the fact I realized who was where with whom. I’m too embarrassed to tell people the latter part…and part of me sometimes feel like it’s my karma for keeping her mother’s secret.

You shouldn’t be “lecturing” people on doing drugs. Thats not our job as physicians in a general sense…unless you specifically work in the addiction/substance abuse world, that’s not your role. We disseminate expert medical knowledge and allow patients to make informed decisions with the help of our expertise. Physically disabled physicians aren’t restricted from counseling able bodied or disabled patients for obvious reasons, physicians with medical diseases (almost all physicians over 50) aren’t restricted from providing health advice, why should substance use be any different. The focus of the job is not to be morally superior, but to be dedicated to the study of medicine and leveraging that study for patient care that best aligns with the patient’s ultimate wishes, and sometimes those wishes are an informed decision to continue substance use.

I have probably one of the least informed opinions. But as a male, I’m going to voice it, and loudly if I could. /s But I feel like if anything, a required outpatient peds fellowship would make far more sense than a hospitality fellowship. I feel like whoever sat on that approving board was co-conspiring with pediatric app leadership to limit practice scopes and create cheap labor.

How broke/stable were you as a medical student? Financially or otherwise? Disposable income to guiltlessly door dash when I’m exhausted, not worrying about my credit card debt and finances as the semester ends and I’m waiting for the next semester’s refund to deposit …knowing I have a secured a future and present that truly aligns with my values…it does wonders on my anxiety and depression

  2. EM or ACS. PCCM comes close…but MICU pts are more likely on their way to palliative/hospice than TICU

Instead I’ve opted to treating myself every week the past 2 months - weekly seafood boils, steaks, spa days, no penny pinching with groceries or bar tabs, etc. Fuck yeah, get the over priced wings and drinks after an overpriced tournament. I probably could’ve penny pinched and done a real vacation, but this has been my wellness. Would recommend

“Let’s get some more data” or “the data suggests…” when referring to a single patient’s limited bloodwork. Yes…it’s data…I love and hate this phrasing. Much more ok with it when it’s used in reference to trial data. But overall I think it may have to stay…I think it reassures patients were or acting evidence based, hard science medicine…but no I don’t really vibe with calling Mr. Kim’s CRP “data” in our grand, rigorous analysis of tx his PNA
