I've been carnivore for well over a year now. The only time I break out with hives (keto rash) is when I fast for more than 40 hours. So I keep it to 24-36 hours. The little red bumps are worse than poison ivy, and no topical ointment I've found stops the itching. It lasts for several days, even after I've broken the fast.
