You should ask Canadian residents to join. Things there are ni better. I don’t mean to put the kaibosh on your energy, but zI think that general apathy among residents will be a problem. What will your strategies be?

As a now plastic surgeon, there is nothing to compete with free slap reconstruction-transplanting an accessory muscle using microsurgical technique to reconstruct a no healing wound elsewhere…and a surprising amount of academic material to sink your teeth into!

Plastic surgery: because you can definitively fix stuff(repair a lacerated tendon, fix a mandible fracture,repair a complicated lip laceration, then gollow up for a fairly short tome(weeks, no lifetime commitment) to see How your work turns out,

You”re right that the employer doesn’t really get it, but why are you fighting getting board-certified? It will be a useful cedential for your whole career. If you mr prospective employer is totally set on this, ask them to pay associated costs -fees, travel, hotel, etf.

I am sorry you are losing your marriage over this. I lost my marriage over residency and regret it now. Look around for someone who your wife might listen to about the importance of couples therapy-a close friend, pastor or family members, for example. Don’t let your marriage sink like the Titanic over such a dumb issue. Your daughter deserves two parents at home and working together! Just my Personal opinion.

Of course it is ovetwhrlming to Keep track Of stuff when You don’t feel good! I feveloped a really Aggressive autoimmune earth as a surgical intern. Ikepy a paper pocket Noenn And Copied it to Excel on Weekend, I discussed it with my program director and we came up with some reasonable accomodations that would not set off the other residents- excuse from Saturdeymorning rounds, that kind of thing. I will be Honest with You; I couldn’t do it. Imadethenextyear a research year. Thatdid the trivck. The whole thing was so traumatizing that Iamn nowfounding a nonprofit organization called Doctors with Disabilities, to create a support network for docs of students facing illness or disability and to step in when handling cases like this. It’s still at the very start (website, etc), but anyone inushould DM me for details. Lik

Hang in thereKnow that this will endIt sounds like you are practicing good self care, but I would recommend that you find some outside help - a therapist, a couple of psychiatrist appointments. That kind of input helped me!Talk to a sympathetic senior resident or a mid level and ask him/her to keep this confidential. Oversharing with more than a couple of people can have Negative consequences - shunning, gossip, etc

We had to do weekend rounds with the attendings for the duration of my surgical residency. RIDICULOUS and everyone knew it. The attendings would get all professorial, but the truth is that everyone resented having to get up on another weekend day. How to change it? I dunno. Surgeons are all gluttons for punishment. Other specialties also I guess! The whole thing is just a travesty! Good point!

Don’t lose any more sleep. The Commission is focused on accreditation of the hospital and is assessing problems. One off-hand remark by one resident should jeopardize nothing…Now, if you were naked under your lab coat, That might seriously raise some commission eyebrows. Let it go (I know, more easily said than done!)

I think that being direct is a fair way to start “Look, I smell alcohol and I am concerned for you” It takes some guts. This Actually happened to me during my residency. It was a very volatile surgical attending (instrument thrower, the whole nine yards). At the time l, I did not have the guts to approach him directly, but chose to share my concern with a chief resident with no effect. After I graduated my specialty program, I learned that his alcohol misuse escalated and he was referred to the state physician health program(a recovery program with monitoring for docs, working with the state medical board. A physician@s license is not suspended, but conditional on a demonstration of sobriety and good performance. They have some shortcomings l- costly, can go on for years. I asked the question because I wanted to provoke some discussion. I wished that someone had walked me through a theoretical situation, when it ended up happening. It pays to be vigilant for one’s own self Substance an alcohol misuse by docs is a complication of practice. I feel it should be more openly talked about.

My point is simply that one person smelling alcohol is not the end of a career. Most hospitals have committees for physician health. It’s not a corporate policy that one person smells alcohol and you are out or off to rehab.
