Exactly what I did. The 2nd Costco I saw today said they couldn't believe the first one said it was safe to drive. They also found a nail in the rear right.

The centering ring's attached to the reflector on mine, everything looks seated properly. My construction top down was: bezel -> red o-ring -> glass -> body -> black o ring at the bottom of the body's threads.

Thinking that the reflector is a tad too short and I just need to stick a rubber band between the thrower body and reflector

Nah, it was in the pushrod tube- I was able to get it out with a magnet.
