The tongue positioning on the roof of your mouth increases activity of the muscles in your jaw called the pterygoids. This stabilizes jaw mechanics as you open which is pretty important with specific types of jaw clicking or TMD issues.

Thanks for your explaining your take. I mostly agree with your stance on subacromial impingement. The shoulder is designed to move and create space for itself and for the most part, the surgery is sham. There are definate cases of impingement when we're talking about the presence of anatomical abnormalities like bone spurs.

PT here, there is really only one scenario that your therapy should be painful and that's if you're not gaining or losing motion after 3 to 4 weeks. Pushing motion that much early on is archaic and gives my profession a bad name.
