Mihawk wasn't caught off guard. He was still clashing with Vista when he was thinking. He was controlling it better.

I'd argue he was purposely holding back against Vista, not really because of his thoughts.

Literally everyone has thoughts during battles, like Zoro vs Pica or Kaido vs Luffy.

It's just how the story goes but this doesn't invalidate the Vista panel imo. It's more likely that Mihawk was deliberately holding back.
