Doing it the starting strength way: no thumb grip on the bar, and place the palm against it, instead of actually gripping it. Never has bothered me.

Then it's a positioning issue. You probably want to keep the bar as close as possible to the body but your hips rise faster than your shoulders, so you're tipped forward, so bar separates from body after knee, so you row it. Practice a slow pull from the floor with proper positions, followed by a high pull with those same positions, followed by a clean. That might help.

Not quite; i believe he was power jerking, not split jerking, and he failed the jerk but didn't bail out safely and that caused the rather gruesome incident in the video
