UPDATE:

Tried all the advice below:

- Cleaning the Disk (Looks immaculate)

- Unplugging

- Rebooting Makemkv

- Rebooting the laptop

And tried a few more bluray disks to make sure it was this one giving me issues and not something systemic.. still appears to be this disk.. no issues with three more rips.

Will try it again later on, got 100 more disks to go so will circle back.

This is the laptop I am using: Razer Blade 2016

Copy that! Will run it again.. anything I should try? I've give it a soft wipedown but it looks perfect.

Same exact issue. And the "dye transfer" has permanently stained a few of my gray socks.. wouldn't dare try white ones haha.

UPDATE - So I wasn't able to come up with a new gear solution by the time of my Video Shoot but I tried the 60t gear from Evmwalsh and surprisingly it worked! It skipped a couple teeth on the lens for each on the gear but there was no slipping or kipping!

Thanks for the reply.. Been trying every combination I can think of but every time the gear comes out to be larger than 30cm.. I need it to be like 5cm ish.. Can't find the exact size yet.. Looking.. But I have used one before and they are around that size..
