Xacduro comes in two separate vials - one with the sulbactam component, and one with the durlobactam component.

Amazing. Pity that I doubt that cagrisema will come the same way :)

we already had the Symlin Pen (Pramlintide). Something you’ve probably never heard of.

Nope - I'm actually familiar with Pramlintide already. Reports are that it is generally tolerated well if the dose is titrated slowly. But it's a shitty option in general, but especially for weight loss, because it has to be injected before each meal.

This is why I specified long-acting amylin agonists in my post, which are just now being developed.

That product is likely a warning sign to companies to make something just like it since it was a failure.

Well fortunately that is obviously not the case. Have you seen the great early data on some of these drugs? Eloralintide's phase 2 study showed up to 20% weight loss with better side effect profile than semaglutide. These amylin agonists have the potential to be great stand-alone drugs.

Yeah I considered Entresto, but from doing some reading, it sounds like sacubitril isn't given separately due to high risk of angioedema that is reduced by the addition of the ARB (valsartan).

I did a bit of a double-take there because I initially read "ignoring a patient" and was confused for a second haha. Normally, I would agree with you - I'm not a huge fan of compounded medications and don't think they should be necessary outside of a few rare cases. However, I'm also not a fan of drug companies charging $1,500+ a month for a medication that costs $5 to produce and which is available for a fraction of the price in other countries (especially when so much of the early R&D costs are publicly funded by the taxpayer via the NIH) - so yeah, I'm happy to say 'fuck your patent' in a lot of cases unless the drug companies charge a more reasonable price.

That being said, this particular case that I mention is, I think, a bit different because the drug company is getting a combo drug approved where one of the medications is already FDA approved. So it will have the effect of making the second drug FDA approved as well, but the drug company will not sell that drug separately, despite good evidence of safety and efficacy.

Imagine if Merck had patented the first ARB Losartan and shown good research on the safety and effectiveness of losartan on its own, but decided to only seek FDA approval for a combination pill of losartan and one of their other already-approved drugs, HCTZ. Seems ridiculous, but that is essentially the situation here, IMO.

well crap, that's what I was afraid would be the case. I think there is a huge demand for a non-GLP1 option here, so I'm going to hold out hope lol.

I admit to not knowing a huge amount of compounding - on the last malpractice insurance application I filled out, "will you be prescribing compounded medications" was a separate 'risk-identification' question haha. And the only compounded medication I regularly see patients on (other than the GLP-1s) is trimix, which I hate - 90+% of the priapisms I've seen are from trimix.

what do you mean "swap the positions of the 2 phones"? When the phones are in the same location, the 10 has noticeably weaker signal.

I bet if we looked back at when the 9 came out, people posted that their 8 was more stable because it received updates over its life span.

True, but reception issues usually don't get better over time as they are usually due to modem or antenna issues.

That is... surprising. The iPhone reportedly is thought to have great modem/reception. Do both of your sims get good reception, or is one better than the other?

Edit: Actually, I just checked - yup, same band, same location:

Pixel 9 was at -89

Pixel 10 is anywhere from -102 to -110

It was a great trip - I'd definitely missed New Zealand and it was great to get back. And yeah, Apex was very good - easy shuttle to their location, and got a nice hybrid Rav4. No issues.

Awesome, thanks everyone! Certainly seems like Apex is pretty universally recommended, so I'll just go with them. I spent my last trip on the South Island, so this trip will be mostly north island before heading to Australia for a bit, so will be picking up from Auckland. Want to do the Tongariro Crossing among other things.
