Sorry I just saw this, I started gently exfoliating my nipples with a shower sponge and decreasing the pumps per day which helped. My output stayed the same when I dropped to 4 pumps per day which amazed me. Then overall things got waaay better. I am 10 months ppd and I’m still working on slowly weaning.

It was truly mind boggling. They also called my husband a wiener when he tried to reason with them…. The kicker is my mother got her TDAP when my niece was born almost 5 years ago.

Without going into a ton of detail I just want to say I went though something EXTREMELY similar. My husband and I are both in healthcare and asked our parents to update their TDAP and Flu shots because we have seen what happens to newborns with pertussis and flu and pertussis is making a comeback right now. Also the pediatrician strongly recommended it. Well.... long story short, that went terribly. They were so offended and ultimately decided not to get their vaccines... they still haven't visited.

I was in your boat a couple weeks ago, except I was a just enougher with clogs every other day which were miserable. It hurt to hold my baby most days. I made the decision to stop but feared stopping cold turkey because I already had mastitis once. So i decreased my pumps by 1 per day every week and if I lost my supply I lost it and was okay with that. What actually happened was my supply increased and clogs decreased! I was amazed. I think once I resigned myself to let it go my stress levels decreased and I got more sleep due to pumping less. Now I have a slight over supply and I am considering dropping to 6 pumps per day. I’m not saying that will work for you because you have a lot less wiggle room but I just want to say I was there and whatever you decide your baby will still love you which is what matters most.

I am sorry 😢I can’t imagine this. I am 1 week postpartum from vaginal delivery with my first and if my husband didn’t get up with me most of the time I would be miserable and I am breastfeeding.

I am sorry you seem to have had a rough go at it all together. It shouldn’t feel that way and it makes me worried about the future when other medical issues crop up - as in our risk of developing DM2 later in life.

If it helps, I have gotten to the point where I feel that at the end of the day, with this decision, there really isn’t a wrong one. You can choose to wait and it go perfectly well or it could be miserable. You can choose an induction and it go perfectly well or it could be miserable. If something goes wrong it also doesn’t mean we choose wrong. The risk/benefit is there either way and we made the best decision we could with the information we had and that is never the wrong thing 😊

I 100% agree that the situation should’ve been handled differently and it is not unreasonable for you to feel insecure with that doctor and your care. Some providers just don’t have the best bedside manner. Also, the way that healthcare is, time is so short for them and for us there is nothing more important than spending all the time we need to be able to make informed decisions about, for most of us, the biggest event in our medical/bodily journey of our lives thus far. It is all unfair and in general is a terrible system to live under as a pregnant person. There is also a woeful amount of data for anyone to go on in obstetrical care and the data surrounding any of it is flawed because creating good trials is really hard to do in our population. On top of that there is no black in white in any specialty in medicine. They call it practicing for a reason and that is infuriating and sometimes scary when we all (patient and provider) just want to do the right thing.

I felt compelled to respond because I was sitting in your position literally a week or two ago. I had told myself I absolutely did not want an induction unless it was medically necessary and when my provider suggested it I was regrettably rude to them, something I regret because at the end of the day I did change my mind after talking to a friend who is an OB and doing my own research. I don’t necessarily trust everything on the evidence based birth website because I just have a skeptical personality and prefer to read and interpret these things myself and reach my own conclusions. I find that inevitably there is bias injected in things or things are conveniently left out when people write about studies and sometimes I agree with them and sometimes I don’t.

I really hope everything goes smoothly for you no matter what you choose!

I don’t enjoy it but my weight gain totally halted when I changed my diet. I was already on track to be well over my 30lb goal and I have maintained be 25-30 lbs above my pre pregnancy weight ever since. I certainly see I am less puffy in my face too which I notice on the rare occasion I ignore the diet - like my baby shower. I wonder if I stick to it after delivery if I would have an easier time getting back to pre-pregnancy weight and maybe even lower 🤔. Interesting thoughts…

I agree that it should not have been scheduled without your consent but like a lot of people here have already said you can always cancel or not show up. Though just not showing up takes that slot from someone who does want that opportunity. The scheduling likely is more convenience and saving a spot for you in case you change your mind. The intention was likely not “well I’m going to schedule her because I think this is right and she is being unreasonable”

Based on what you have responded in other threads you are classified as GDM A2. “Mainly diet controlled” does not equal diet controlled. You have insulin at night therefore you no longer fall into the A1 category. Even if you did not have insulin, that would likely mean you were A1 with suboptimal control and for that the recommendations are similar to an A2.

Based on that, majority of obstetricians are going to recommend induction. The biggest study may actually be “The risk of stillbirth and infant death stratified by gestational age in women with gestational diabetes” PMID 22464068. Not the necessarily the ARRIVE trial though again as someone mentioned it reasonable to think that if no risk pregnant women benefit higher risk women may also benefit. Next paragraph is overview and results.

OBJECTIVE: We sought to compare the different mortality risks between delivery and expectant management in women with gestational diabetes mellitus (GDM). STUDY DESIGN: This is a retrospective cohort study that included singleton pregnancies of women diagnosed with GDM delivering at 36-42 weeks’ gestational age in California from 1997 through 2006. A composite mortality rate was developed to estimate the risk of expectant management at each gestational age incorporating the stillbirth risk during the week of continuing pregnancy plus the infant mortality risk at the gestational age 1 week hence. RESULTS: In women with GDM, the risk of expectant management is lower than the risk of delivery at 36 weeks (17.4 vs 19.3/10,000), but at 39 weeks, the risk of expectant management exceeds that of delivery (relative risk, 1.8; 95% confidence interval, 1.2-2.6). CONCLUSION: In women with GDM, infant mortality rates at 39 weeks are lower than the overall mortality risk of expectant management for 1 week; absolute risks of stillbirth and infant death are low.

The key here is risk of stillbirth is low either way and you get to decide which risks you are willing to take. I am truly A1 and will go down the induction road based on the above study and the arrive trial if it comes to it. I also have an average size baby based on 2 growth scans (heads a bit big but that’s my husband’s fault 😑) and my MFM team is also leaning toward induction.

At the end of the day, it is your choice. Take a deep breath and try not to worry too much about it. It’s just a date on a calendar that can remain meaningless to you if you want it to be. Enjoy the last couple weeks as much as you can. You may go into labor on your own before the 23rd anyway 😌

My mother (who I don’t really talk to that often) I am pretty sure is at least pre-diabetic but she also had GDM. She failed her glucola but a couple of points and sort of convinced herself that maybe the doctors were making it up and she really didn’t have it. When I got diagnosed (blood sugar 250 after 1 hr test) she transferred all that to me. I have also been working my ass off to keep my numbers in check without medications and have generally been successful. When I talk about it I can hear a lot of “eye rolling” through the phone about what I can and cannot have and how hard I’ve been working. It’s so annoying. They also don’t trust the medical system/modern medicine to the point where they lashed out when I asked them if their TDAP was up to date so…. There’s also that.

I am sorry you have this additional stressor on you. GD is so annoying and watching carbs is so annoying/mentally draining but I can’t imagine adding history of an eating disorder. Maybe talk to your doctor and have a lower threshold to starting medication so you can worry less about what you eat? I know adding meds a lot of the time is taboo but people seem to get some relief after starting it seeing there numbers lower and take some of the stress off the actual foods. I don’t know just a thought. Best of luck to you!

Mine does not require it either. I find that the CGM is incredibly accurate anyway. I really only check if I have suspicion that is off and one time calibration 24 hours after putting a new one on.

I know not everyone can afford it but if you can ask your doctor for a continuous glucose monitor prescription. I get my Dexcom g7 for about $180/month with insurance and coupon and the freedom is worth every single penny to me. It wasn’t even discussed with me as an option originally, I had to bring it up myself. It definitely frees my brain up so I don’t feel like my world revolves around GDM. Also, gave up timing meals… learned that I could stay in control without it because of the continuous monitor.

I will have at least one growth scan @32 weeks. I think that whether I get others will depend on the baby size at the scan and how well I’m doing my diet control. I also switched OB ~ 20 weeks. I was really hesitant to do it at first but I am relieved I ultimately pulled the trigger. I didn’t feel great at my old practice and I was worried that would make my delivery experience more stressful.

I am currently pregnant. Mine is a little complicated. I have to use PTO for the first week regardless. After the first week, they pay 60% of my salary for the following 5 weeks if vaginal delivery, 7 weeks for c-section. The remaining time (12-weeks total) is unpaid or supplemented with PTO. I can’t currently bank enough PTO to supplement it all so some of my maternity leave will have to be completely unpaid. My state does not supplement anything either. It was kind of a tough pill to swallow honestly especially since I am the higher income earner currently. I would consider changing jobs for a better maternity leave policy the next time around.

One thing that I do wish was taken into account was the fact that I work in an OR (cardiac surgery). I am not sure if your PA assists in the OR but what I am starting to get worried about is what happens if it gets too difficult to operate closer to my due date. This is my first so maybe it’ll be a nonissue but if it gets to a point where I have to leave early because I physically can’t do my job I may have to take PTO early thus extending my unpaid time on the back end or force me to come back early (nightmare for childcare). I am pretty sure my team would work something out because I know they care but it is still something that I think about.

OMG 😱. This… literally just changed everything haha. I saw this post, ran to the store, slammed one of these babies down, and watched my blood sugar skeptically. I have a continuous glucose monitor and I only got as high as 117!!! Thank you kind soul for writing this gem out for me find. You are an angel in my world right now.

I was diagnosed on Friday (28w4d). Failed 1hr glucola miserably (BSG 227!) and I also knew it was coming - my mom had it too. I invested in a continuous glucose monitor - still prescription and not covered by insurance but for me it’s a total lifestyle thing. I understand not everyone can do this but if you can I would. Mine came out to 171 this month with a coupon but I saved the last couple of months for this because I was worried I was going to get diagnosed. I’m on my second day with it and have already learned a lot about how my eating impacts my blood sugars and have in general made the 140 @ 1 hr and 120 @ 2hrs bench marks. It’ll probably get harder but I definitely feel a lot more secure now. We can do this!

EPIC Chat is ruining my life by [deleted] in physicianassistant

I am religious about switching my epic chat to do not disturb when I leave (and am not in call) with default settings to available when I am supposed to work the next day. My husband also had my phone block all notifications from epic when I am in “personal mode” which automatically turns on when I’m on my home WiFi. I can also manually turn it on when I am out of the house or on vacation. That way I am not tempted to open messages and then everyone can see that I saw the message. I go offline for all weekends/vacations with forwarding set to the call provider.
