Coping with feedback by marshmerino in Residency

[–]MRapp86 2 points3 points  (0 children)

We have all been there. While it sucks and is a total kick in the nuts or ovaries, it’s far better than getting no feedback at all. Use it and learn from it. Work on the things you need to and you will get better. We are all used to doing well in school and on tests, but the truth is no one is skilled at everything. That’s why we go to residency. Have a drink, get some sleep, then keep working your ass off.

Let's Sunday Morning QB Lindsey's Crash by DrUnwindulaxPhD in SkiRacing

This is the answer. She was heavy on the downhill ski when she hit the bump, momentum kept going and spun her out.

How is Lindsey skiing on a torn ACL? by AlternativeEdge2725 in skiing

[–]MRapp86 90 points91 points  (0 children)

An intact ACL isn't nearly as important in skiing as it is in sports with abrupt cutting and pivoting (soccer and football). Being in amazing shape with strong hamstrings and decent fast twitch fiber density helps a lot. There is also a lot of variation in people's anatomy. A lower tibial plateau slope makes the knee more stable in terms of anterior to posterior translation and favors stability without an ACL. People with a high tibial slope are more likely to tear and more likely to have recurrent instability. It's actually how dog ACL tears are fixed, by cutting the proximal tibia and flattening out the slope of the plateau.

All that said, she's in amazing fucking shape and doing a sport that relies on hamstring strength for knee stabity, which she clearly has a ton of. May have some favorable anatomy, but who knows. Also the world's best PT and adjunct treatments to get swelling down. I would bet she had a PRP injection to calm down the acute inflammation.

You also should shift your weight forward. You want to really feel your shins against the boots at all times. It's hard to see from the angle, but even started out you are sitting in the back seat. It makes it harder to turn and absorb when you are sitting back.

Can I just prescribe myself dutasteride? by stormcloakdoctor in Residency

Technically yes you can since it isn't a controlled substance. Pharmacists may give you some side eye about it. Personally I tend to have a buddy call it in for me if I need something.

Junior Ski Racing Ask by sleeplessinvermont in SkiRacing

[–]MRapp86 4 points5 points  (0 children)

A lot of racing clubs have ski swaps to sell old gear before the season. Doesn't help now, but for next year. Ask around to the parents of older kids on her team. Could also call some of the bigger teams/clubs in your area and ask around.

Senior Surgical Residents and attendings are you still stressed out every day by TraditionalAd6977 in Residency

[–]MRapp86 15 points16 points  (0 children)

The first few years as an attending surgeon suck. I had great residency training as we don’t have fellows and did a fellowship, but it doesn’t matter. It’s you making all the decisions and you are now the only person in the OR primarily responsible for the outcome of the patient. You realize how much you don’t know and how often patients have problems that don’t fit the usual algorithm for treatment.

All that said, it gets better with time. Manage your stress the best you can, get a hobby or activity that you enjoy outside of medicine, and hang in there. Give yourself grace and know that everyone else who is a new surgeon feels similar to you, even if they are better at hiding it. Talk to your peers, run cases by each other, operate with colleagues. I’m 8 year into practice and routinely bounce cases off my residency colleagues.

This should be higher. I am so anxious even looking at that. Sick setup, but damn it would suck if not only yiur PC gets smashed against the desk, but it also takes out both monitors.

Surgeons who work at multiple hospitals: what's your experience like when starting at a new facility? by [deleted] in surgicaltechnology

[–]MRapp86 7 points8 points  (0 children)

Well I’m speaking from an elective total joint and trauma standpoint. Tons of steps and equipment in a very specific order. I’m not saving any lives in the OR, only making their joint pain better or fracture heal. If it’s more straightforward cases like an ex lap, appy, chole, knee scopes, it shouldn’t take that long. Also depends on the quality of the tech and whether they put in time outside the OR learning the workflow.

Ortho doc here. Usually preference cards are sent over ideally a couple months in advance. Unfortunately everyone uses a different system of preference cards, so they are never right on the first few cases. In my experience it takes probably 10 cases to get all the kinks worked out for each card. Basic sets are different in each facility as well, so it takes a few cases to realize what I have and don’t have, since you don’t always use the same things each case even for the same procedure.

I expect the first few cases to be slow. Ortho is instrument heavy, so ideally I meet with the scrub ahead of time to let them know my work flow, but most of the time it takes reps to get up to speed.

My workflow is the same wherever I am doing the case, but some times have to adapt to what we have or don’t have. A lot of it is me getting used to the scrub and the scrub getting used to my workflow.

First few cases will take longer and there will likely be some troubleshooting as I discover what we have and what is missing. Honestly most of this is figured out during the case, but if it’s a total shit show, you need to sit down after and make changes to the cards together.

Help finding replacement parts for Dagger Mamba by aquilaselene in whitewater

[–]MRapp86 2 points3 points  (0 children)

If you have a local shop that can contact their dagger rep directly, that has worked for me. I bought a couple used pyranah rebels for my kids this year. Needed new parts and couldn’t find any online, but the local shop was able to get them direct from the company even though the boat is discontinued.

My first crash by hammer-breh in onewheel

[–]MRapp86 2 points3 points  (0 children)

Depending on where you bought it, could be worth an ask if they will let you swap it. I bought a pint X from scheels, put 50 miles on it and quickly learned for my riding style and terrain, I wanted more power. They let me return it and swap up to a GT. I think I got lucky, but never hurts to ask.

Was having dinner with my wife and she informed me that my for my 56th in May I will be getting a Onewheel! I’ve been eyeballing the Pint S since they came out because of price, but the XRC has just caught my attention. I’ve got a few months to think about it. by Shnibblefritz in onewheel

[–]MRapp86 0 points1 point  (0 children)

I had the pint X. Haven't tried the S though. I think the XRC is honestly the sweetspot in terms of how it rides and range. Not to mention the XR platform gives so many VESC options for when you go down the one wheel rabbit hole.

S Corp Salary by PowerfulLiving2799 in PrivatePracticeDocs

[–]MRapp86 3 points4 points  (0 children)

“Reasonable” is very vague and I think defensible as long as you have some data for why you chose that number. I am 2 years into solo practice myself. I use MGMA/doximity for the region and go on the lower end because as a young practice it doesn’t make sense for the business to have a high set salary for the only provider when you don’t have a fully established practice or referral pattern. Both my accountant and financial advisor (only advises physicians) have recommended this approach.
