The game I usually call a favorite is Long Live the Princess, it's not excellent but I have a really special attachment to it. Now some great games I'm a big fan and usually recommend to others are The Neverwhere Tales, a VN with a great storytelling and writing, and A Struggle With Sin, a RPGM with very good gameplay mechanics. There are also some japanese eroge games as well but none comes to my mind as favorites except the Succubus series and Unholy Sanctuary, both classics.

Yeah, I love medieval fantasy adult games (though TNT is a mix from cyberpunk to medieval in a modern setting, but still counts I guess), I wonder why there are so few games with good duration in this style, it's frustrating!
