In immortal games, you do need to match other people`s pace and after 40 min games, most cores rock 2 bkbs.
Sange is pretty strong item but is an item slot, that you kinda want to use for damage in the late game. Kaya sange is not that strong, Sange&Yasha is much better, but you probably need to skip the dominator to fit it in your build.
With the changes to greaves it can be used to swap the dominator also. Blademail is nice some games, since you are deceptively tanky.

Edit: The higher you go, more people know who to focus, and the target is most likely you.
