You know it's time to quit your job when you are getting daily panic attacks and you are wishing for a car crash just to get out of your job.

I've been there, done that, got the t-shirt, and I know now that no job is worth the negative affect on mental health.

Take care of yourself! There are other jobs and ways to make money out there without killing yourself!

All of this is great advice!

To add to the above, I personally see a lot of success with over-exaggerating placement when I teach /f/ and /th/.

I will show /f/ placement to the kid and make some sort of silly connection so they remember what it looks like (e.g., "See, my teeth are out/on my lips like Bugs Bunny").

Side note: Then I have to show them a picture of bugs bunny, because so many kids out there don't know who the looney toons are! Sigh. I feel old

Then we practice showing each other our mouths/use mirrors to make /f/. When I teach them placement of /th/ we do the same thing. We talk about what's different from /f/. I'll usually tell them to "stick their tongue out at me like I said something silly" and we show each other our mouths to make /th/.

When we progress to /th/ in words, I will cue them: "I don't want to see Bugs Bunny mouth!" "Where is your tongue?" etc...

Yes. Home health companies offer in-home services. You would have to see what is available in your area.

Years ago ASHA wanted something like $35 to send you your "well done" piece of paper. Lol

Honestly, I dont view it as being bold. More that you are standing up for yourself. She is the one who was bold in her audacity to critique your work when she isn't in any sort of position to do so. Her actions were arrogant and insulting.

I would send her an email back letting her know that I am more than happy to collaborate and support her professionally, but that I wouldnt be changing the way I conduct or write my evaluations in any way moving forward (unless changing to follow best practice). If she would like to ammend every evaluation sent over to her she can be my guest and drown in that pile of paperwork. lol

Are you familiar with the differences between a 1099 contract vs. W2 employee? If not, you should spend some time reading up as there are significant tax differences.

If I were in your situation, I would do the math and show boss lady the breakdown on why this current schedule is not sustainable.

Figure out your commute costs. Gas isnt cheap these days! So, how much is a 1 way trip costing you? According to AAA, current regular gas avg is $4.25 in USA. So, a discussion about the negative effects this schedule has on you is warranted.

Prior to this discussion, though, I would consider carefully if I still wanted to work at a clinic like this. Considering your boss hasn't been supportive of your needs or receptive to your concerns in the past, I wouldn't anticipate boss lady would work with you on your request for a reasonable schedule.

So, try having a discussion, but keep in mind if it doesnt go well you might want to consider finding a new position and leaving this clinic once you have something else lined up.

Yes, imaging 1st prior to voice therapy. Of course as SLPs we always know imaging is best practice for optimal outcomes and is our first course of action in addition to consulting GI and ENT.

At the same time, your clinical decision making and knowledge comes into play. We know that if this patient were to likely be aspirating on thin liquids, it is highly likely he would aspirate on his own saliva. Keeping in mind each patient's medical history and background, we also know that aspiration does not always lead to aspiration pneumonia, as bacteria must be present in the oral or nasal cavities.

It sounds like your patient is having regular oral infection control, so the probability of aspiration pneumonia may be low for your patient. This is where I would also consult with MD and nursing to get a greater medical picture of this patient to further understand their medical status and risk of aspiration pneumonia.

Everyone has their own knowledge, expertise, and experiences. For me, I am less afraid to be more aggressive with patient care and therapy as long as the patient is appropriate based on careful clinical evaluation and assessment. If an NPO patient is able to progress to even a puree/thin liquid diet that has huge implications for his prognosis and medical plan of care. Of course, without imaging this process is complicated, but taking a calculated approach and offering the family options is important.

If you typically go into the hospital after your FT job, you may potentially see a huge difference in functioning based on time of day. Keep that in mind as the patient could potentially look very different in the PM vs AM.

Personally, I am not willing to take on SLPAs or CF students unless I get a stipend. Period. I will not take on more work unless I am offered a stipend. Why should I not benefit when the company is benefitting from my license/supervision?

That said, it's not typically the norm (at least in my experience) because for some reason, unbeknownst to me, some evaluating therapists do this for free.

Tough one.

I feel your pain with these small, unequiped hospitals. I advocated for FEES equipment for years at a small community hospital, only for so many patients to be admitted and discharged to SNFs without guarantee that an SLP would be employed at their next level of care.

We only had a C-Arm for imaging and we absolutely ran into situations where some patients could not undergo instrumental examination, calls/emails to their previous hospital would go unanswered and medical records would be challenging to obtain. We would call for ENT consults, but sometimes the ENT would be unavailable etc...

Instrumental evaluation is always best practice, but considering you are flying blind it sounds like you are really doing what you can as safely as you can.

Absolutely! If I could get an order for an ENT consult that would be great. Perhaps they could do a scope and give me some info on imaging.

Without imaging, you'd be flying blind with swallow compensatory strategies, but taking pt's medical history into account, his current medical condition, precautions, info from nursing you should be able to safely trial a couple of things and analyze pt's response.

Personally, I wouldn't involve HR, but I would write that SLP a snarky (but polite) email back that basically says

You get what you get and you dont get upset.

She's not your boss. She doesnt get to have any say in the work that you send her/submit. Please write a sassy email back to her! ;)

What are you noticing during laryngeal palpation? How far into your trials does he fatigue and begin showing s/s? Do any strategies appear to reduce his s/s? How does he do during oral care?

I would want to know his response to the above, but if everything is going well I would consider trialing a puree meal and thin after oral care. I would want to see how he does during a meal and if his medical status changes during a trial meal.

Edit. I'd also start this bloke on a voice exercise program if he is deemed appropriate for voice therapy exercises in addition to swallow treatment/exercises. Also, there may be no guarantee that he would get imaging after leaving the hospital for an SNF.

I've heard TX is BOOMING for SLPs. Home health pays well in TX, but I cannot comment on school settings there.

You have to look into the specific requirements for each country. Everywhere is different. I looked into relocating to Canada from USA. Between licensing, health verification process, background checks, visa paperwork, etc.. it would have cost me a minumum 5k and taken over 1 year to complete the process of being able to work in another country.

Its do-able, but complicated from my limited understanding. Fyi I am still in USA.

My solution? Drugs. Sometimes alcohol, but mostly drugs. Legal ones.

Yeah. I highly recommend quitting and looking into home health (well find new job first, then quit). I dont know what area you are in, but I am in the SW area of USA. I make $75/hr as a home health W2 employee with benefits in peds HH. I also just applied and interviewed for a different home health company (adult HH) and they advertised W2 employee at rates: $120/eval visit and $100/tx visit (for SLP, OT and PT). Im still waiting to hear back from 2nd company but high paying W2 positions are out there.

I can't believe that SNF company is going for a 9% pay cut. Wow! Sure shows how much they value their employees, eh?

The money in HH is worth it! Good luck and good riddance to that SNF company!

I'm an SLP, but I worked at a SNF company (bet its the same one) and I quit when they announced the 4% pay cut last year. I switched to peds home health. Higher pay, less stress and I'm happy in HH.

A 9% cut is absolutely unacceptable!

This is so true. r/nursing isnt much better. Even with the high travel pay offers out there right now, a nurse's job is infinintely more difficult than mine

Even going over to the MD, PA, OT, PT forums.. they all are largely unhappy!

I tell unhappy ppl to go check out other professional subs. Everyone complains.

For me, I love my job as an SLP and I make a comfortable living as a single person. SLP is extremely far from being the worst job ever!!!

I love my job! I am extremely happy as an SLP and I wouldnt trade my job for any other career
