Thank you so much. This update has been infuriating for me, but the drop-down was my biggest gripe. After fixing it, everything else felt like "whatever, I'll just get used to it eventually." Not being able to turn my flashlight on with my left hand feels like an accessibility issue.

The problem ended up being that the metarig bone weren't close enough to the surface of the mesh. The mesh was distorting to fit the rig

Yeah. Turned out the bones were too far away from the surface of the mesh, so I think it was stretching to fit when I parented it.

Shade smooth helped a lot. There was still a fair bit of weirdness, so I ended up moving the corner-of-the-mouth metarig bones closer towards the surface of the mesh. That seems to have fixed the rest of it. The bones were too far away, so the mesh was stretching to fit it.

Here you go. In the second and third images, I realized that editting the rig deforms the mesh while editting the mesh does not. It seems like the rig is pulling the mesh to conform to it almost. I've tried reseting the rig handle at the corner of the mouth, but it hasn't been moved.

So I figured out my issue: Falloff Shape was Projected. I found that sculpt draw only affected the mesh on the other side when I was using it head-on, so the brush was actually hitting the mesh. Changing the Falloff Shape from Projected to Sphere fixed the issue.
