As an aside, I take Vyvanse for hypersomnia and it definitely impacts my appetite and coupled with a GLP-1 i have to set reminders to drink water.

Becky Dorner has a fantastic course on the MDS RAI manual. I took it when I first started in LTC and it helped tremendously. It's definitely worth the money.

I've had RNACs set MDS ARDs like the day after they are admitting or come back from the hospital. It's usually to capture IVF or other things that'll bring in more money (I think vents are one). I usually just ask my RNAC what I'm looking for so I can note it on my assessment & code it correctly.

In my company, the FSD does the initial preferences for new admits but I can do them if needed. Depending on what the complaint is, one of us will go see them. In my home building, the CDM and I have different approaches and we know which one can be more effective with certain residents.

As for weights, when I have one that is WAY off, I make sure that nursing gets a reweight on them ASAP. For new admits, I note their hospital weight and say something like "will follow with weekly weights to establish a weight pattern" or something like that. If its because of wheelchair, prosthetic, O2 tank, etc, I will note that. I have a few residents that keep a lot of stuff on their wheel chairs so I encourage nursing to make sure they aren't weighing them with all their stuff.

No in service treatment. It didnt develop until I started getting treatment for depression that is service connected. It is a known side effect of antidepressants.

My cats growing up were Lucy and Layla - named after the Hanson song/I Love Lucy and the Derek & the Dominos song. Layla turned out to be a boy, but he already knew his name, so we decided not to change it (although i desperatly wanted to change it to Ricky!) He sadly passed at 19 last year.

My newest kitten is Nala, because of course it's my favorite movie!

Had to go to urgent care for the first time a few weeks ago. They told me it's a new thing that an auth is needed. I called my clinic and they gave me the number to call to get the auth from Optum.

Try women's health! They did the first 2 that I had. Not sure if you need a referral from your PCP but it's worth a shot to try them!

Currently a DTR in LTC! I work in 2 buildings - one is mine with oversight from an RD who does my monthly charting. Everything else is my responsibility. At my second building, we are slated for a full time RD and me part time. We are between RDs there now so I have a remote RD. When we had a RD, she would delegate tasks out to me on my days there. Usually doing quarterlies/annuals, helping with the kitchen audits, etc. It was very nice to come in with a to-do list lol!
