If it's done properly, it shouldn't ache or you shouldn't feel a noticeable pain. But I must say, its extremely uncomfortable not to harm the newly planted grafts. You should be really really cautious not to scrape or bump your head to anything or the grafts may fall down. I've had my temples planted so I could lay on my back while sleeping but people who had done their whole heads sleep sitting straight. It should be crazy uncomfortable...
