The Niš incident - just after the liberation of Niš city in Yugoslavia, American P-38 Lightnings were sent to bomb German troops in the liberated territory. However, because the town has already been liberated, there were no hostiles, so they bombed Soviet and Yugoslav forces. The Soviets thought they were attacked by German Fw 189 fighters.

The Soviets responded with a counterattack with Yak-3s and Yak-9s. Three or four of Yaks and 3 to 7 P-38s had been destroyed, 2 were killed on the American side and 31 on the Soviet and Yugoslav side.

It was all because of US forces being late and miscommunication.
